摘要
目的分析常规体外受精(c-IVF)中无原核(0PN)、单原核(1PN)来源冻融单囊胚移植的妊娠结局及新生儿情况。方法纳入接受c-IVF后冻融单囊胚移植的255例不孕患者,并根据囊胚来源,将患者分为0PN组(n=27)、1PN组(n=19)和双原核(2PN)组(n=228)。比较3组患者的妊娠结局和新生儿情况。结果1PN组的胚胎着床率、临床妊娠率、持续妊娠率、活产率均低于其他两组,但差异均无统计学意义(均P>0.05)。3组的早期流产率、异位妊娠率以及新生儿出生时孕周、出生时体重、性别构成比差异均无统计学意义(均P>0.05)。所有新生儿均无重大畸形发生。结论在c-IVF中,当正常2PN胚胎数量不足时,移植0PN、1PN来源冻融单囊胚能够获得可接受的临床妊娠率和活产率,在患者充分知情同意的情况下,可以考虑移植。
Objective To analyze the pregnancy and neonatal outcomes of frozen-thawed single-blastocyst transplantation from non-pronucleus(0PN)and single pronucleus(1PN)in conventional in vitro fertilization(c-IVF).Methods A total of 255 infertile patients,who received frozen-thawed single-blastocyst transplantation after c-IVF,were enrolled.According to the source of blastocysts,the patients were divided into 0PN group(n=27),1PN group(n=19)and two pronuclei(2PN)group(n=228).The pregnancy and neonatal outcomes of the three groups were compared.Results The embryo implantation rate,clinical pregnancy rate,ongoing pregnancy rate and live birth rate in the 1PN group were lower than those in the other two groups,but the differences were not statistically significant(all P>0.05).There were no statistically significant differences in early abortion rate,ectopic pregnancy rate,gestational age at neonatal birth,birth weight or sex composition among the three groups(all P>0.05).No major malformation occurred in all newborns.Conclusion In c-IVF,when the normal 2PN embryos are insufficient,the acceptable clinical pregnancy rate and live birth rate can be obtained by transplanting 0PN and 1PN-derived frozen-thawed single blastocyst,and transplantation can be considered in the case of well informed consent of patients.
