基于四维时空关联成像技术建立早中孕期胎儿心胸比正常参考值范围

Establishment of normal reference range of fetal cardiothoracic ratio in early and middle pregnancy based on four-dimensional spatiotemporal image correlation technique

摘要 目的基于四维时空关联成像(STIC)技术建立早中孕期胎儿心胸比(心胸横径比、周长比和面积比)的正常参考值范围。方法采用四维STIC技术对675例孕11~23^(+6)周正常单胎孕妇进行检查,获取胎儿标准四腔心切面并测量心胸横径比、周长比和面积比。分析心胸比与孕周的相关性,并采用正态近似法制定不同孕周胎儿心胸比的正常参考值范围。结果共获得609例胎儿的STIC数据,STIC容积图像数据获取成功率为90.22%(609/675)。609例胎儿的心胸横径比、周长比及面积比分别为0.43±0.04、0.49±0.04、0.22±0.04,均与孕周均呈正相关(均P<0.05)。对入组的609例胎儿数据进行统计分析,得到不同孕周胎儿心胸横径比、周长比及面积比的95%正常参考值范围。结论孕早期正常胎儿心胸横径比、周长比和面积比随着孕周增长而增加。成功建立了基于STIC技术的早中孕期胎儿心胸比的正常参考值范围,可为诊断和评估胎儿心胸疾病提供参考数据。 Objective To establish the normal reference range of fetal cardiothoracic ratio(cardiothoracic transverse diameter ratio,perimeter ratio and area ratio)in early and middle pregnancy based on the four-dimensional spatiotemporal image correlation(STIC)technique.Methods A total of 675 normal singleton pregnant women with 11 to 23^(+6) gestational weeks were examined by using the four-dimensional STIC technique.The standard four chamber view of fetus was obtained and the cardiothoracic transverse diameter ratio,perimeter ratio and area ratio were measured.The correlation between cardiothoracic ratio and gestational week was analyzed.In addition,the approximate normal distribution method was employed to determine the normal reference range of fetal cardiothoracic ratio at different gestational weeks.Results The STIC data of 609 fetuses were obtained,and the success rate of STIC volume image data acquisition was 90.22%(609/675).The cardiothoracic transverse diameter ratio,perimeter ratio and area ratio of 609 fetuses were 0.43±0.04,0.49±0.04 and 0.22±0.04,respectively,all of which positively correlated with gestational week(all P<0.05).A statistical analysis was conducted on the data of 609 fetuses recruited to obtain the 95%normal reference ranges of cardiothoracic transverse diameter ratio,perimeter ratio and area ratio of fetuses at different gestational weeks.Conclusion In early pregnancy the normal fetal cardiothoracic transverse diameter ratio,perimeter ratio and area ratio rise with the increasing gestational week.The normal reference range of fetal cardiothoracic ratio in early and middle pregnancy based on STIC technique is established successfully,which can provide reference data for the diagnosis and evaluation of fetal cardiothoracic diseases.
