摘要
目的分析不同血流量血液灌流联合血液透析治疗尿毒症的临床疗效。方法 80例尿毒症患者,采用随机数字表法分为低血流量组和高血流量组,每组40例。低血流量组患者给予200 ml/min血流量血液灌流联合血液透析治疗,高血流量组给予250 ml/min血流量血液灌流联合血液透析治疗。比较两组肾脏疾病生活质量简表评分,治疗前后β2-微球蛋白(β2-MG)、尿素氮(BUN)、肌酐(Scr)水平,治疗效果,凝血发生率。结果高血流量组肾脏疾病生活质量简表评分为(86.25±3.21)分,高于低血流量组的(74.56±2.91)分,差异有统计学意义(P<0.05)。治疗前,两组β2-微球蛋白、BUN、Scr水平比较差异无统计学意义(P>0.05);治疗后,两组β2-微球蛋白、BUN、Scr水平均低于治疗前,且高血流量组低于低血流量组,差异有统计学意义(P<0.05)。高血流量组总有效率高于低血流量组,差异有统计学意义(P<0.05)。高血流量组凝血发生率低于低血流量组,差异有统计学意义(P<0.05)。结论高血流量(250 ml/min)血液灌流联合血液透析对尿毒症的治疗效果确切,可有效改善肾功能,减少凝血发生,提高患者生存质量。
Objective To analyze the clinical efficacy of different blood flow hemoperfusion combined with hemodialysis in the treatment of uremia.Methods A total of 80 uremia patients were divided into low blood flow group and high blood flow group according to random numerical table,with 40 cases in each group.The low blood flow group was treated with 200 ml/min blood flow hemoperfusion combined with hemodialysis,and the high blood flow group was treated with 250 ml/min blood flow hemoperfusion combined with hemodialysis.The kidney disease quality of life score,β2-microglobulin(β2-MG),blood urea nitrogen(BUN),creatinine(Scr)before and after treatment,therapeutic effect,and incidence of coagulation were compared between the two groups.Results The kidney disease quality of life score of high blood flow group was(86.25±3.21)points,which was higher than(74.56±2.91)points of low blood flow group,and the difference was statistically significant(P<0.05).Before treatment,there was no statistically significant difference inβ2-microglobulin,BUN,Scr levels between the two groups(P>0.05).After treatment,theβ2-microglobulin,BUN and Scr of the two groups were lower than those before treatment,and high blood flow group was lower than low blood flow group,and the difference was statistically significant(P<0.05).The total effective rate of high blood flow rate group was higher than that of low blood flow group,and the difference was statistically significant(P<0.05).The incidence of coagulation of high blood flow group was lower than that of low blood flow group,and the difference was statistically significant(P<0.05).Conclusion Combination of high blood flow(250 ml/min)hemoperfusion and hemodialysis shows affirmative effect in the treatment of sepsis,and it can effectively improve the renal function,reduce occurrence of coagulation,and improve the quality of life of patients.
