摘要
目的分析长寿区交通伤害死亡率及疾病负担变化趋势,为开展交通伤害综合防治策略和措施提供依据。方法收集分析2011-2019年重庆市长寿区交通伤害死亡个案信息(ICD-10编码V01-V99)。采用SPSS 22.0软件统计分析交通伤害死亡率、标化死亡率、伤残调整生命年(DALY)、早死所致的寿命损失年(YLL)、疾病所致伤残引起的健康寿命损失年(YLD)、DALY率、YLL率和YLD率等指标。率的比较采用卡方检验,率的趋势变化分析采用年度变化百分比(APC),检验水准为α=0.05。结果重庆市长寿区交通伤害标化死亡率由2011年的23.21/10万下降至2019年的8.72/10万,APC为-10.95%,变化趋势有统计学意义(P<0.001)。历年交通伤害男性死亡率均高于女性,差异均有统计学意义(P<0.01)。0~14岁组及15~64岁组死亡率均呈下降趋势,65岁及以上组保持较高水平。交通伤害YLL率、YLD率、DALY率分别由2011年的10.66‰、1.16‰和11.81‰下降至2019年的3.37‰、0.37‰和3.73‰,APC分别为-13.50%、-13.41%、和-13.50%,变化趋势均有统计学意义(P<0.001)。DALY率男性高于女性。结论重庆市长寿区交通伤害死亡率和疾病负均低于重庆市水平,呈下降趋势,应继续做好重点人群交通伤害的防治。
Objective To analyze the trend of traffic injury deaths in Changshou district so as to provide evidence and recommendations for making policy for comprehensive prevention of traffic injury.Methods Information about the cases of traffic injury(ICD-10:V01-V99) from 2011 to2019 were collected and analyzed.The mortality,age standardized mortality rate by Chinese population(ASMRC),years of life lost with premature death(YLL),years lived with disability(YLD),disability adjusted life years(DALY) and the rates of YLL,YLD and DALY were calculated by using software SPSS 22.0.Chi-square test was used to compare the mortality of traffic injury.The annual percentage change(APC) was used to analyze the trend.Results The ASMRC of traffic injury decreased from23.21/105 in 2011 to 8.72/105 in 2019 in Changshou district,and the APC was-10.95%(P<0.001).The mortality of traffic injury in males was significantly higher than that in females over the years(P<0.01).The mortality of 0-14-year-old group and 15-64-year-old group showed a decreasing trend,while the mortality of 65-year old and above group remained at a high level.The rates of YLL,YLD and DALY decreased from 10.66‰,1.16‰,11.81‰ in 2011 to 3.37‰,0.37‰,3.13‰ in 2019;the corresponding APCs were-13.50%、-13.41%、-13.50%,respectively(P <0.001).The rate of DALY in males was higher than that in females.Conclusion The mortality and disease burden of traffic injury were lower than those in Chongqing,which showed a decreasing trend.Traffic injury prevention and control among the high-risk population should be enhanced.
