腹腔镜胆囊切除患者术前睡眠质量与术后疼痛的相关性探讨

The Correlation Between Preoperative Sleep Quality and Postoperative Pain in Patients with Laparoscopic Cholecystectomy

目的:探讨腹腔镜胆囊切除手术患者术前睡眠质量与术后疼痛的相关性。方法:选取2018年8月至2019年8月蚌埠市第一人民医院普外科收治的腹腔镜胆囊切除手术患者120例作为研究对象,根据WHO制定的《深睡眠质量评估表》将患者分为睡眠质量尚可组70例,睡眠质量较差组50例。术后6 h、12 h、24 h、48 h用视觉模拟评分法(VAS法)评估患者疼痛程度,比较2组患者术后VAS评分、疼痛评分下降率,Spearman相关方法分析睡眠质量与VAS评分相关性。结果:1)睡眠质量尚可组术后6 h、12 h、24 h、48 h的VAS评分均低于睡眠质量较差组,且疼痛评分下降率明显更高,差异有统计学意义(P<0.05);2)Spearman相关性分析睡眠质量与VAS评分呈正相关(r=0.324,P=0.001)。结论:腹腔镜胆囊切除手术患者术前睡眠质量越好,术后疼痛越轻,术前睡眠质量评分与术后疼痛VAS评分呈正相关。

Objective:To investigate the correlation between preoperative sleep quality and postoperative pain in patients undergoing laparoscopic cholecystectomy.Methods:A total of 120 cases with laparoscopic cholecystectomy from August 2018 to August 2019 in General Surgery Department of the First People's Hospital of Bengbu were selected as the research objects.According to the WHO's Deep Sleep Quality Assessment Form,they were divided into tolerable sleep group(n=70)and the poor sleep quality groups(n=50).Visual Analog Scale(VAS score)was used to evaluate the pain degree of patients at 6 h,12 h,24 h and 48 h after surgery.The decreased rates of pain score and VAS score in 2 groups were compared.Spearman correlation method was used to analyze the correlation between sleep quality and VAS score.Results:1)VAS scores at 6 h,12 h,24 h and 48 h in the tolerable sleep group were lower than those in the poor sleep group,and the decrease rate of pain score was significantly higher.The difference was statistically significant(P<0.05);2)Spearman correlation analysis showed a positive correlation between sleep quality and VAS score(r=0.324,P=0.001).Conclusion:The better preoperative sleep quality of patients undergoing laparoscopic cholecystectomy,the less postoperative pain,and the preoperative sleep quality score is positively correlated with postoperative pain score.
