高值医用耗材集中带量采购评审分组规则比较

Comparative Study on Grouping Rules in Centralized Purchasing with Quantity of High-value Medical Consumables

目的:为各级医保部门建立更加科学公平的高值医用耗材集中带量采购评审分组规则提供参考。方法:采用文献分析法梳理试点地区的高值医用耗材集采方案,采用比较分析法对试点地区的总体实施情况与分组规则进行研究。结果:国家和27个省份已进行了高值医用耗材带量采购试点,涉及12种产品,高值医用耗材价格平均降幅分别为92.58%、56.92%。现行的高值医用耗材带量采购评审分组规则分为5种,以功能属性、市场占有率、产地属性、产品价格、组套分组作为分组规则的地区数分别为7个、3个、3个、2个、2个。结论:高值医用耗材带量采购试点成效显著,但品种同质化问题突出。按照市场占有率、产地属性、产品价格、组套分组进行分组不能有效实现“同质同组竞价”,不利于投标主体公平竞争。按照功能属性分组能在较大程度上实现“同质同组竞价”。建议高值医用耗材带量采购评审分组以功能属性为主要分组规则,同时兼顾各评审单元的竞争度,以充分均衡降价与质量保障的关系。

Objective:To provide references for the establishment of more scientific and fair grouping rules in centralized purchasing with quantity of High-value medical consumables for medical insurance departments at all levels.Methods:The literature analysis was used to sort out the centralized purchasing schemes of High-value medical consumables in the pilot areas,and comparative analysis was used to study the overall implementation situation and grouping rules in the pilot areas.Results:The country and 27 provinces had carried out the pilot project of purchasing with quantity of High-value medical consumables,involving 12 kinds of products,and the average price drop was 92.58%and 56.92%,respectively.The current grouping rules in purchasing with quantity of High-value medical consumables could be divided into 5 categories,among which the number of regions with functional attribute,market share,origin attribute,product price and set grouping as grouping rules was 7,3,3,2 and 2,respectively.Conclusion:The pilot of purchasing with quantity of High-value medical consumables has achieved remarkable results,but the homogenization of varieties is prominent.According to the grouping rules of market share,origin attribute,product price and set grouping,the goal of same quality and same group bidding can't be effectively realized,which is not conducive to fair competition of bidding subjects,but grouping by functional attributes can guarantee the same quality and same group bidding of consumables to a large extent.It is suggested that the functional attributes should be taken as the main grouping rule in purchasing with quantity of High-value medical consumables,and the competition degree among each unit should be taken into account to fully balance the relationship between price reduction and quality assurance.
