基于数据挖掘的国医大师治疗溃疡性结肠炎用药规律研究

Study on Medication Rules in Treatment of Ulcerative Colitis by National TCM Masters Based on Data Mining

目的基于数据挖掘方法分析首批国医大师治疗溃疡性结肠炎(UC)的用药规律,为临床辨治UC提供参考。方法检索中国知识资源总库(CNKI)、中国学术期刊数据库(万方数据)建库至2019年12月31日收录的文献,并手工检索《国医大师专科专病用方经验》等著作中30位首批国医大师治疗UC的医案和验方,运用古今医案云平台V2.2.1结合IBM SPSS Statistics 20.0软件对UC中医证候、治法及用药频次、性味归经、配伍、核心处方进行统计分析。结果共纳入17位国医大师的149首处方。UC常见中医证候为湿热内蕴、脾虚湿热,常见治法为清热化湿、健脾益气,常用中药为健脾化湿药,常用炮制方法为炒炭。关联规则分析得到7组常用药对,其中党参-白术频次最高。因子分析得出10个有效公因子,其中公因子1为健脾化湿组,公因子2为清肠止痢组。结论国医大师治疗UC以健脾化湿为基本治法,巧用清肠止痢之品,不忘兼顾脾肾。

Objective To analyze the medication rules of the first batch of TCM masters for treating ulcerative colitis(UC)based on data mining methods;To provide a reference for clinical syndrome differentiation and treatment of UC.Methods Literature included in CNKI and Wanfang Data was retrieved from the establishment to December 31,2019,and medical cases and records about UC treated by the first batch of 30 TCM masters were manually retrieved from Guoyi Dashi Zhuanke Zhuanbing Yongfang Jingyan,etc.TCM syndromes,treatment methods,medication frequency,properties,taste,compatibility,and core prescriptions were analyzed by using Cloud Platform of Ancient and Modern Medical Record V2.2.1 combined with IBM SPSS Statistics 20.0 software.Results Totally 149 prescriptions of 17 TCM masters were included.The common TCM syndromes of UC were damp-heat inside,spleen-deficiency and damp-heat.The common TCM treatment methods were heat-clearing and dampness-reducing,spleen-tonifying and qi-strengthening.The commonly used Chinese materia medica is tonifying spleen and dampness reducing medicine,and the commonly used processing method was carbonizing by stir-frying.Analysis of association rules revealed 7 groups of commonly used drug pairs,with the highest frequency of co-occurrence of Codonopsis Radix-Atractylodis Macrocephalae Rhizoma.Factor analysis yielded a total of 10 valid common factors,in which common factor 1 was the spleen-tonifying and dampness-reducing group,and common factor 2 was intestineclearning and diarrhea-relieving group.Conclusion The basic treatment of TCM masters for UC is tonifying spleen and reducing dampness,flexibly combining with medicine for clearing intestines and relieving diarrhea,taking into account the spleen and kidney.
