基于数据挖掘的吕文良治疗原发性肝癌用药规律研究

Study on LüWenliang’s Medication Rules for Primary Liver Cancer Based on Data Mining

目的利用数据挖掘技术分析吕文良教授治疗原发性肝癌用药规律,并与其临证思想进行印证。方法收集2017年1月-2019年9月中国中医科学院广安门医院吕文良教授门诊原发性肝癌患者医案103份,采用古今医案云平台V2.2.2进行用药统计分析、聚类分析、关联规则分析和复杂网络分析。结果共纳入处方135首,涉及药物176味。药性以寒、微温、平为主,药味以苦、甘、辛为主,主归脾、肺、肝经。高频药物聚类得到5个聚类组;关联规则分析得到5个常用药对,核心药物为黄芪和焦山楂、焦六神曲、焦麦芽;复杂网络分析显示,核心处方为黄芪、防风、黄连、黄芩、焦山楂、焦六神曲、焦麦芽、法半夏、茯苓、陈皮、姜厚朴、白芍、白花蛇舌草、半枝莲、熊胆粉、芒硝。结论本研究得出的用药规律与吕教授治疗原发性肝癌养肝健脾、调气和血、解毒化瘀、扶正祛邪的临证经验相符。

Objective To analyze the medication rules of Professor LüWenliang in the treatment of primary liver cancer by using data mining technology;To confirm with his clinical thoughts.Methods Totally 103 medical records of patients with primary liver cancer from the outpatient clinic of Professor LüWenliang,Guang’anmen Hospital of Chinese Academy of Chinese Medical Sciences from January 2017 to September 2019 were collected,and the ancient and modern medical case cloud platform V2.2.2 was used for drug statistical analysis,cluster analysis,association rule analysis and complex network analysis.Results Totally 135 prescriptions were included,involving 176 kinds of Chinese materia medica.The properties were mainly cold,lukewarm,flat,and the tastes were mainly bitter,sweet,and pungent.The meridians were mainly attributed to the spleen meridian,lung meridian,and liver meridian.Five cluster groups were obtained by high-frequency medicine clustering;five common TCM medicinal pairs were obtained by association rule analysis,and the core Chinese materia medica were Astragali Radix and Jiao Hawthorn,Jiao Liu Shenqu,Jiao Malt.Complex network analysis showed that the core prescriptions were Astragali Radix,Saposheikovize Radix,Coptidis Rhizoma,Scutellariae Radix,Jiao Hawthorn,Jiao Liu Shenqu,Jiao Malt,Pinelliae Rhizoma,Poria,Citri Reticulatae Pericarpium,Magnoliae Officinalis Cortex,Paeoniae Radix Alba,Scutellariae Barbatae Herba,Xiongdan Powder,and Natrii Sulfas.Conclusion The medication rules obtained in this study is consistent with Professor Lü’s clinical experience in treating primary liver cancer,nourishing the liver and spleen,regulating qi and blood,detoxifying and removing blood stasis,and strengthening the body resistance to eliminate pathogenic factors.
