PTA患者关节液中IL-1、TGF-β及TNF-α的表达及临床意义

Expressions and clinical significance of IL-1,TGF-βand TNF-αin the joint fluid of patients with post-traumatic arthritis

目的分析创伤性关节炎(PTA)患者关节液中白细胞介素-1(IL-1)、转化生长因子-β(TGF-β)及肿瘤坏死因子-α(TNF-α)的表达及临床意义。方法选取2017年8月至2020年8月本院收治的192例PTA患者作为研究对象,根据膝骨性关节炎严重性指数(index of severity for knee joint osteoarthritis,ISOA)分为中度组(n=45)、严重组(n=46)、非常严重组(n=50)、极严重组(n=51)。同时纳入155例健康志愿者作为对照组。比较各组间IL-1、TGF-β、TNF-α水平差异,采用Pearson法分析PTA患者IL-1、TGF-β、TNF-α水平与ISOA的相关性。结果研究组IL-1、TGF-β、TNF-α水平均高于对照组,差异有统计学意义(P<0.05)。不同严重程度者IL-1、TGF-β、TNF-α水平比较:中度组<严重组<非常严重组<极严重组,差异均有统计学意义(P<0.05)。PTA患者IL-1、TGF-β、TNF-α水平与ISOA呈正相关关系(P<0.05)。结论IL-1、TGF-β、TNF-α水平在PTA患者异常升高,三者与患者病情严重程度呈显著正相关性,其表达情况可作为评估病情程度的依据。

Objective To analyze the expressions of interleukin-1(IL-1),transforming growth factor-β(TGF-β)and tumor necrosis factor-α(TNF-α)in the joint fluid of patients with post-traumatic arthritis(PTA)and analyze the clinical significance.Methods A total of 192 patients with PTA admitted to our hospital from August 2017 to August 2020 were selected as the research subjects.According to the index of severity for knee joint osteoarthritis(ISOA),they were divided into a moderate group(n=45),severe group(n=46),very severe group(n=50),extremely severe group(n=51).At the same time,155 healthy volunteers were included as a control group.The differences in the levels of IL-1,TGF-β,and TNF-αbetween the groups were compared.The Pearson method was used to analyze the correlation between the levels of IL-1,TGF-β,and TNF-αin PTA patients with ISOA.Results The levels of IL-1,TGF-βand TNF-αin the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Comparison of levels of IL-1,TGF-β,and TNF-αin patients with different severity levels:moderate group<severe group<very severe group<extremely severe group,the differences were statistically significant(P<0.05).The levels of IL-1,TGF-β,TNF-αin PTA patients were positively correlated with ISOA(P<0.05).Conclusion The levels of IL-1,TGF-β,and TNF-αare abnormally elevated in PTA patients,and the three are significantly positively correlated with the severity of the patient.s condition,and their expression can be used as a basis for assessing the severity of the condition.
