E-cadherin、COX-2及TRACP5b与原发性骨巨细胞瘤患者复发的关系 被引量:2

Relationship between the levels of E-cadherin,COX-2 and TRACP5b and the recurrence of patients with primary giant cell tumor of bone

摘要 目的分析上皮型钙黏蛋白(E-cadherin)、环氧化酶-2(COX-2)及抗酒石酸酸性磷酸酶5b(TRACP5b)与原发性骨巨细胞瘤患者复发的关系。方法选取2018年1月至2018年12月本院病理科65例原发性骨巨细胞瘤患者的肿瘤组织及肿瘤旁正常骨组织分别作为癌组织组、癌旁正常组织组,另选取同期在本院体检的73例健康者设为对照组。比较各组E-cadherin、COX-2及TRACP5b的表达,分析E-cadherin、COX-2及TRACP5b与原发性骨巨细胞瘤患者各病理参数的相关性,采用多元Logistic回归分析影响原发性骨巨细胞瘤患者复发的危险因素。结果 E-cadherin及COX-2在癌组织中阳性表达率显著高于癌旁正常组织,原发性骨巨细胞瘤患者TRACP5b水平明显高于对照组,差异均有统计学意义(P<0.05)。肿瘤越大、临床分期及Jaffe分级越高的原发性骨巨细胞瘤患者其TRACP5b、E-cadherin及COX-2蛋白的阳性表达率均越高(P<0.05)。患者复发率为32.30%(21/65)。肿瘤≥5 cm、JaffeⅢ级、手术方式:单纯刮除、E-cadherin蛋白阳性、COX-2蛋白阳性、TRACP5b水平异常升高者复发率高(P<0.05)。Jaffe分级为Ⅲ级、手术方式为单纯刮除、E-cadherin蛋白阳性、COX-2蛋白阳性、TRACP5b水平异常升高为影响原发性骨巨细胞瘤患者复发的危险因素(P<0.05)。结论 E-cadherin、COX-2蛋白及TRACP5b水平在原发性骨巨细胞瘤患者中呈高表达状态,且其水平变化与患者复发密切相关,临床上检测其水平对监测患者复发具有重要意义。 Objective To analyze the relationship between the levels of E-cadherin,COX-2 and TRACP5 b and the recurrence of patients with primary giant cell tumor of bone.Methods The tumor tissue and normal bone tissue adjacent to the tumor of 65 patients with primary giant cell tumor of bone from January2018 to December 2018 in the pathology department of this hospital were selected as the cancer tissue group and the normal tissue group adjacent to the cancer.In addition,73 healthy people who were examined in this hospital during the same period were selected as the control group.The expressions of E-cadherin,COX-2 and TRACP5 b in three groups were compared.The correlation between the levels of E-cadherin,COX-2 and TRACP5 b and the pathological parameters of patients with primary giant cell tumor of bone was analyzed.Multivariate logistic regression analysis was used to analyze the risk factors affecting the recurrence of patients with primary giant cell tumor of bone.Results The positive expression rates of E-cadherin and COX-2 in cancer tissues were significantly higher than those in normal tissues adjacent to cancer,and the TRACP5 b level of patients with primary giant cell tumor of bone was significantly higher than that in the control group,the differences were statistically significant(P<0.05).The larger the tumor,the higher the clinical stage and the higher the Jaffe grade,the higher the positive expression rates of TRACP5 b,E-cadherin and COX-2 protein in patients with primary giant cell tumor of bone(P<0.05).The recurrence rate of patients was 32.30%(21/65).Tumors≥5 cm,JaffeⅢgrade,surgical methods:curettage alone,E-cadherin protein positive,COX-2 protein positive,and abnormally elevated TRACP5 b levels have a high recurrence rate(P<0.05).Jaffe gradeⅢ,simple curettage,positive E-cadherin protein,positive COX-2 protein,and abnormal increasing of TRACP5 b were risk factors affecting the recurrence of patients with primary giant cell tumor of bone(P<0.05).Conclusion The levels of E-cadherin,COX-2 protein and TRACP5 b are highly expressed in patients with primary giant cell tumor of bone,and their changes are closely related to patient recurrence.Clinical detection of their levels is of great significance for monitoring patient recurrence.
