根管治疗后两种口腔修复体的计算机辅助设计与计算机辅助制作铸瓷修复体的效果

Clinical efficacy of cast porcelain restorations of computer aided design and computer aided manufacture of two kinds of dental restorations after root canal therapy

目的探究对根管治疗后的磨牙行两种不同形式的计算机辅助设计与制作(CAD/CAM)铸瓷修复体修复后的临床效果。方法选取2018年6月至2019年6月在江门市口腔医院接受磨牙根管治疗后需接受缺损修复的患者100例为研究对象,对其进行回顾性分析,根据修复方式的不同进行分组,观察组接受髓腔固位冠修复,对照组接受玻璃纤维桩核和全冠修复,每组各50例。所有患者的修复体材料均由计算机辅助设计制作(CAD/CAM)。比较不同修复方式的临床疗效、修复后边缘密合性、完整性、继发龋、牙龈炎症、对颌牙磨耗评分以及并发症发生率情况。结果两组治疗效果比较,差异无统计学意义(P>0.05);观察组边缘密合性、完整性、继发龋、牙龈炎症以及对颌牙磨耗评分显著低于对照组,差异有统计学意义(P<0.05);随访1年,两组患者并发症总发生率比较,差异无统计学意义(P>0.05)。结论对接受磨牙根管治疗的患者行髓腔固位冠修复效果更佳,可有效提高修复的完整性以及边缘密合性,并能缓解牙龈炎症等症状,可在临床广泛推广应用。

Objective To investigate the clinical efficacy of two different forms of CAD/CAM cast porcelain restorations on molars after root canal therapy.Methods A total of 100 patients requiring defect repair after molar root canal therapy in Jiangmen Municipal Stomatological Hospital from June 2018 to June 2019 were selected as the research objects,and they were analyzed retrospectively.According to different repair methods,they were divided into the observation group(n=50)and the control group(n=50).The observation group was treated with medullary retention crown restoration,while the control group was treated with glass fiber post core and full crown restoration.All patients’restoration materials were made by computer aided design and manufacture(CAD/CAM).The effects of different restoration methods,marginal tightness after restoration,integrity,secondary caries,gingival inflammation,dental abrasion score of opposite jaw and incidence of complications were compared.Results In therapeutic efficacy,there was no significant difference between the two groups,without statistically significant difference(P>0.05).The scores of marginal tightness,integrity,secondary caries,gingival inflammation and dental abrasion score of opposite jaw in the observation group were significantly lower than those in the control group,with statistically significant differences(P<0.05).After follow-up of 1 year,there was no statistically significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion It is better to repair the patients who were treated with root canal therapy of molars with medullary retention crown restoration,which can effectively improve the integrity and marginal tightness of the restoration,and relieve the symptoms such as gingival inflammation.Therefore,it can be widely promoted and applied in clinic.
