肩关节磁共振在冈上肌损伤中的诊断价值研究

Diagnostic value of shoulder MRI in supraspinatus muscle injury

目的探究肩关节磁共振在冈上肌损伤中的具体诊断价值。方法选择2019年1月至2019年12月我院收治的85例肩袖冈上肌肌腱损伤患者为研究对象。对患者使用1.5T核磁共振检查,分析结果。结果在所有受试者中,均存在冈上肌肌腱形态信号异常之表现。在此其中斜冠状位诊断确定为3级者17例、2级者27例、1级者41例。横断位检查3级者共计16例、2级者共计11例、1级者共计58例;斜矢状位检查3级者11例、2级者8例、1级者66例。将临床好转案例归为挫伤1级;2级为部分撕裂;3级为完全撕裂。在此其中1级共计40例,2级24例,3级21例。57例病患同时存在三角肌以及肩峰下滑囊积液之情况;有29例患者存在盂唇损伤的不良情况,54例病患存在喙突下滑囊积液。钩形肩峰共计31例、弧形肩峰共计32例、平型肩峰共计22例。骨挫伤患者共计41例。肩峰出口情况:5 mm以下者共计16例、5~10 mm者共计54例、10 mm以上者共计15例。斜矢状位、横断位以及斜冠状位扫描具体结果针对于冈上肌腱受损程度判定存在统计学意义。结论针对于肩袖冈上肌肌腱受损患者来讲,为其实施肩关节磁共振成像(MRI)扫描能够明确显示出具体的受损程度范围,同时也可实现MRI分级。其对于医生为患者制定临床治疗方案来讲有着不可替代的作用。该法值得进一步在临床中推广。

Objective To explore the specific diagnostic value of shoulder MRI in supraspinatus muscle injury.Methods Eighty-five cases of supraspinatus rotator tendon injury in our hospital from January 2019 to December 2019 were selected as the research subjects.The patients were examined with 1.5T MRI and the results were analyzed.Results In all subjects,there were abnormal signals of supraspinatus tendon.There were 17 cases of grade 3,27 cases of grade 2 and 41 cases of grade 1.There were 16 cases of grade 3,11 cases of grade 2 and 58 cases of grade 1 in transverse view,11 cases of grade 3,8 cases of grade 2 and 66 cases of grade 1 in oblique sagittal position.In the statistical analysis,the clinical improvement cases were classified as grade 1 contusion,grade 2 partial tear and grade 3 complete tear.There were 40 cases of grade 1,24 cases of grade 2 and 21 cases of grade 3.There were 57 patients with deltoid muscle and acromion bursa effusion,29 patients with pelvic lip injury,54 patients with coracoid process effusion.There were 31 cases of hook shaped acromion,32 cases of curved acromion and 22 cases of flat acromion.There were 41 cases of bone contusion.Acromial outlet:16 cases were below 5mm,54 cases were between 5-10mm,and 15 cases were more than 10mm.The results of oblique sagittal,transverse and coronal scans were statistically significant in judging the degree of supraspinatus tendon injury.Conclusion For the patients with rotator cuff supraspinatus tendon injury,MRI scan of shoulder joint can clearly show the specific extent of damage,and can also achieve MRI grading.It plays an irreplaceable role in making clinical treatment plans for patients.This method is worthy of further clinical promotion.
