某综合医院内外科住院患者质子泵抑制剂使用合理性分析

Analysis on application rationality of proton pump inhibitor for inpatients in internal and surgical departments of a general hospital

目的通过对某综合医院内外科住院患者质子泵抑制剂(proton pump inhibitors,PPIs)的使用合理性进行比较分析,为临床合理用药提供参考。方法收集该院2018年7月1日~2019年6月30日间出院并使用过PPIs的住院患者2327例,采用卡方检验比较内外科PPIs口服制剂和注射制剂的用药频度,以及对不合理使用PPIs的差异进行分析。结果①外科PPIs的使用金额明显高于内科;内外科PPIs的使用均以注射制剂为主,存在过度使用注射剂的问题,外科在注射制剂用药频度上的表现更高。②外科PPIs不合理使用率(83.5%)高于内科(70.5%),差异有统计学意义(P<0.001)。③内外科PPIs不合理用药的类型发生率差异有统计学意义(P<0.05)。结论医院应对PPIs的临床使用进行专项管理,制订相应的指导原则,提高其临床用药合理性。

Objective To compare and analyze the rationality of application of proton pump inhibitors(PPIs)on inpatients in internal and surgical inpatients of a general hospital,so as to provide references for the rational use of drugs in the clinic.Methods A total of 2327 inpatients who discharged from hospital between July 1,2018 and June 30,2019 and used PPIs were collected.Chi-square test was used to compare the defined daily doses(DDDs)of PPIs oral preparation and injection preparation in internal and surgical departments,and the difference of irrational use of PPIs was analyzed.Results①The cost of PPIs in surgical departments was obviously higher than that of in the internal departments;the use of PPIs in internal and surgical departments was mainly injection preparation,there was the problem of excessive use of injection,and surgical departments had higher injection preparation DDDs performance.②The unreasonable prescription rate of PPIs in the surgical departments(83.5%)was higher than that in the internal departments(70.5%),and the difference was statistically significant(P<0.001).③There were statistically significant differences in the incidence of types of unreasonable medication in the internal and surgical departments(P<0.05).Conclusion The hospital should conduct special management on the clinical use of PPIs,formulate corresponding guiding principles,and improve the rationality of clinical medication.
