第三版PEP-3孤独症评估量表在ASD患儿早期筛查及疗效评价价值

The Value of the Third Edition of Pep-3 Autism Assessment Scale in Early Screening and Efficacy Evaluation of Children with ASD

目的探讨在孤独症谱系障碍(ASD)患儿早期筛查中,自闭症儿童心理教育评核(第三版)(PEP-3)的应用价值,以及针对ASD患儿实施结构化教学训练的临床效果。方法选取我院2018年1月至2019年12月接诊的疑似ASD儿童368例为研究对象,运用婴幼儿孤独症筛查量表(M-CHAT)、PEP-3量表对疑似ASD患儿进行筛查,并以MCHAT量表阳性检出率作为评价标准,计算PEP-3量表在ASD筛查中的信效度。针对ASD患儿实施结构化教学训练,并对比干预前后PEP-3量表评估结果。结果本组368例疑似ASD儿童中,M-CHAT量表筛查确诊阳性ASD患儿78例(21.20%),PEP-3量表评估结果为(30.81±16.75)分;PEP-3量表的Cronbach’α系数为0.84,ROC曲线下面积(AUC)结果为0.841,以40分为临界值,敏感度为87.18%,特异度为84.83%;结构化教学实施3个月后,78例ASD患儿PEP-3各项指标评估结果均有显著提升(P<0.05)。结论在ASD患儿早期筛查中,PEP-3量表具有较高的应用价值,是有效的筛查工具,且可评价结构化教学训练效果。

Objective To explore the application value of pep-3 in the early screening of children with autism spectrum disorder(ASD),and the clinical effect of structured teaching and training for ASD children.Methods 368 children with suspected ASD in our hospital from January 2018 to December 2019 were selected as the research objects. The children with suspected ASD were screened with M-CHAT and pep-3,and the reliability and validity of pep-3 in ASD screening were calculated with the positive detection rate of M-CHAT as the evaluation standard. The structured teaching training was carried out for children with ASD,and the evaluation results of pep-3 scale before and after the intervention were compared. Results Among 368 suspected ASD children,78 cases(21. 20%) were diagnosed as positive ASD by M-CHAT scale,and the evaluation result of pep-3 was(30. 81±16. 75) points;cronbach’α coefficient of pep-3 scale was 0. 84,area under ROC curve(AUC) was 0. 841,with 40 points as the critical value,the sensitivity was 87. 18%,and the specificity was 84. 83%;after three months of structured teaching,78 cases of ASD were found The results of pep-3 were significantly improved(P<0. 05).Conclusion In the early screening of children with ASD,pep-3 is an effective screening tool and can evaluate the effect of structured teaching and training.
