CT与磁共振成像在强直性脊柱炎骶髂关节病变中的诊断价值分析 被引量:5

摘要 目的分析CT与磁共振成像(MRI)在强直性脊柱炎骶髂关节病变诊断中的应用价值。方法于我院2017年11月至2019年12月收治的强直性脊柱炎骶髂关节病变患者40例,所有患者均给予CT与MRI检查,比较2种方法诊断强直性脊柱炎骶髂关节病变的效果。结果MRI对关节面侵蚀、关节面增生硬化、关节面下骨质囊变的检出率高于CT检查(P<0.05),2种方法的关节软骨肿胀、关节间隙增宽或狭窄检出率比较差异无统计学意义(P>0.05)。CT对0级强直性脊柱炎骶髂关节病变的检出率显著高于MRI,MRI对1级病变的检出率高于CT,经统计学比较,差异有统计学意义(P<0.05)。2种检查方法对2、3、4级骶髂关节病变的检出率比较,差异无统计学意义(P>0.05)。结论强直性脊柱炎骶髂关节病变诊断采用CT与MRI,临床诊断价值高,但与CT相比,MRI更有助于检出患者细微病变,能够为临床诊治强直性脊柱炎骶髂关节病变提供重要依据。 Objective To analyze the value of CT and MRI in the diagnosis of sacroiliac joint lesions in ankylosis spondylitis.Methods Forty patients with ankylospondylitis sacroiliac joint disease admitted to our hospital from November 2017 to December 2019 were selected,and all patients were given CT and MRI examinations to compare the efficacy of the two methods in the diagnosis of ankylospondylitis sacroiliac joint disease.Results the detection rate of MRI on articular erosion,articular hyperplasia and sclerosis,and subarticular bone cyst was higher than that of CT(P<0.05).The detection rates of swelling of articular cartilage,widening or narrowing of joint space were compared between the two methods(P>0.05).The detection rate of level-0 ankylospondylitis sacroiliac joint lesions by CT was significantly higher than that by MRI,and the detection rate of level-1 lesions by MRI was higher than that by CT.After statistical comparison,showed statistically significant difference(P<0.05).The detection rates of grade 2,3 and 4 sacroiliac joint lesions by the two methods were compared,and the difference was not statistically significant(P>0.05).Conclusion CT and MRI are of high clinical value in the diagnosis of sacroiliac joint lesions in ankylost spondylitis.However,compared with CT,MRI is more conducive to the detection of subtle lesions in patients,which can provide an important basis for the clinical diagnosis and treatment of sacroiliac joint lesions in ankylost spondylitis.
