摘要
目的探讨高分辨率彩色多普勒超声在甲状腺癌筛查中的作用。方法回顾分析我院2018年12月至2019年12月收治的甲状腺癌患者48例,各患者术前均接受高分辨率彩色多普勒超声检查,将检查结果与术后病理检查结果进行比较,分析高分辨率彩色多普勒超声检查的意义。结果高分辨率彩色多普勒超声检出率96%低于病理检查检出率100%,但差异无统计学意义(P>0.05),误诊1例,漏诊1例。48例患者接受超声检查,发现双侧4例,单侧44例,肿块共56枚,最小为0.8 cm×0.7 cm,最大为7.6 cm×6.9 cm,共探及52枚结节中有血流,分支紊乱,走行迂曲,粗细不匀,部分增粗,性质为插入性、中心性。本研究中,1级共3枚、2级共18枚、3级共31枚,阻力指数(RI)>0.70,每秒最大峰值速度(PS):15.6~55.6 cm。结论临床在筛查甲状腺癌疾病时,满足指征的状况下,可采用高分辨率彩色多普勒超声进行检查,此方式诊断准确性较高,明确掌握甲状腺癌超声特征,可为临床确定手术方案、选择手术范围提供依据。
Objective To analyze the role of high-resolution color Doppler ultrasound in thyroid cancer screening.Methods Forty-eight patients with thyroid cancer admitted to our hospital from December 2018 to December 2019 were retrospectively analyzed.All patients were received high-resolution color doppler ultrasound examination before surgery.The results were compared with the results of postoperative pathological examination to analyze the significance of high-resolution color doppler ultrasound examination.Results the detection rate of high resolution color doppler ultrasound was 96%lower than that of pathological examination(100%),but the data difference was not statistically significant(P>0.05).1 case was misdiagnosed and 1 case was missed.Ultrasound examination was performed in 48 patients,including 4 patients on both sides and 44 patients on one side.There were 56 lumps in total,with a minimum size of 0.8 cm×0.7 cm and a maximum size of 7.6cm×6.9cm.A total of 52 nodules were found to have blood flow,branch disorder,tortuous movement,uneven thickness and thickness,and partial thickening.In this study,there were 3Ⅰtype,18Ⅱtype and 31Ⅲtype,resistance index(RI)>0.70,and maximum peak speed per second(PS):15.6~55.6 cm.Conclusion High resolution color Doppler ultrasound can be used in the clinical screening of thyroid cancer diseases when the indications are met.The diagnostic accuracy of this method is high,and the ultrasonic characteristics of thyroid cancer can be clearly understood,which can provide a basis for the clinical determination of the operation plan and the selection of the operation scope.
