二维超声及彩色多普勒超声对甲状腺囊性皱缩结节与甲状腺微小乳头状癌的鉴别价值

The value of two-dimensional ultrasound and color Doppler in the differentiation of thyroid cystic shrunken nodules and thyroid micropapillary carcinoma

目的研究二维超声及彩色多普勒超声对甲状腺囊性皱缩结节与甲状腺微小乳头状癌的鉴别价值。方法将2018年11月至2019年11月本院收治的77例甲状腺结节患者作为研究对象,所有患者均接受二维超声及彩色多普勒超声检查,以最终病理为“金标准”。对比分析二维超声及彩色多普勒超声单独检查及联合检查对甲状腺囊性皱缩结节与甲状腺微小乳头状癌的鉴别价值。结果77例受检者中,经病理确诊甲状腺囊性皱缩结节32例,甲状腺微小乳头状癌45例。二维超声联合彩色多普勒超声检查的准确性、特异性、灵敏度、阳性预测值及阴性预测值均较单一二维超声或彩色多普勒超声高,差异有统计学意义(P<0.05);二维超声准确性、特异性、灵敏度、阳性预测值及阴性预测值均略低于彩色多普勒超声,但差异无统计学意义(P>0.05)。结论二维超声联合彩色多普勒超声对甲状腺囊性皱缩结节与甲状腺微小乳头状癌诊断具有较高准确性、特异性及灵敏度。

Objective To study the value of two-dimensional ultrasound and color Doppler in the differentiation of thyroid cystic shrunken nodules and thyroid micropapillary carcinoma.Methods Seventy-seven patients with thyroid nodules admitted to our hospital from November 2018 to November 2019 were selected as the research objects.All patients underwent two-dimensional ultrasound and color Doppler examination,with the final pathology as the"gold standard".To compare and analyze the value of two-dimensional ultrasound and color Doppler single examination and combined examination in the differentiation of thyroid cystic shrink nodules and thyroid micropapillary carcinoma.Results Among 77 subjects,32 were pathologically diagnosed with cystic thyroid nodules and 45 were with papillary thyroid carcinoma.The accuracy,specificity,sensitivity,positive predictive value and negative predictive value of two-dimensional ultrasound combined with color Doppler ultrasound were higher than that of single two-dimensional ultrasound or color Doppler ultrasound,and the difference was statistically significant(P<0.05).The accuracy,specificity,sensitivity,positive predictive value and negative predictive value of two-dimensional ultrasound were slightly lower than color Doppler ultrasound,but the difference was not statistically significant(P>0.05).Conclusion Two-dimensional ultrasound combined with color Doppler has high accuracy,specificity and sensitivity in the diagnosis of cystic thyroid nodules and thyroid micropapillary carcinoma.
