双能量与单能量CTA在颈动脉体瘤诊断中的应用效果比较 被引量:1

摘要 目的比较双能量与单能量CT血管成像(CTA)在颈动脉体瘤诊断中的应用价值。方法抽取2018年3月至2019年5月河南省人民医院收治的80例高度疑似颈动脉体瘤患者,全部患者均先后接受双能量与单能量CTA检查,并以数字减影血管造影(DSA)结果为"金标准",观察双能量与单能量CTA诊断颈动脉体瘤与"金标准"的一致性,并分析两种影像学检查的准确度、特异度、灵敏度;测量比较两种影像学检查不同部位的CT值、噪声、图像质量评分及放射剂量。结果80例高度疑似颈动脉体瘤患者经数字减影血管造影结果证实有63例颈动脉体瘤;双能量与单能量CTA诊断颈动脉体瘤的敏感度、准确度、特异度、阳性预测值及阴性预测值比较,差异未见统计学意义(P>0.05);双能量与单能量CTA的颈总动脉CT值比较,差异未见统计学意义(P>0.05);双能量CTA的颈内静脉CT值高于单能量CTA,竖脊肌CT值噪声和冈上肌噪声均低于单能量CTA,差异有统计学意义(P<0.05);双能量CTA的图像质量评分高于单能量CTA,放射剂量低于单能量CTA,差异有统计学意义(P<0.05)。结论双能量与单能量CTA诊断颈动脉体瘤价值均理想,均与数字减影血管造影诊断结果有较好的一致性,但相比于单能量CTA,双能量CTA的辐射剂量更小,图像质量更好,更具临床应用价值。 Objective To compare the value of dual energy and single energy CT angiography(CTA)in diagnosis of carotid body tumor.Methods Eighty patients with highly suspected carotid body tumor admittde Henan Provincial People’s Hospital from March 2018 to May 2019 were selected,all patients underwent dual energy and single energy CTA examination,and the results of digital subtraction angiography(DSA)was used as the"gold standard".The consistency between dual energy CTA and single energy CTA in the diagnosis of carotid body tumor was observed,the accuracy,specificity and sensitivity of the two imaging examinations were analyzed;the CT value,noise,image quality score and radiation dose of the two imaging examinations were measured and compared.Results Among the 80 patients with high suspected carotid body tumor included in this study,63 cases of carotid body aneurysm were confirmed by DSA;the sensitivity,accuracy,specificity,positive predictive value and negative predictive value of dual energy and single energy CTA in the diagnosis of carotid body tumor was compared,the difference was not significant statistically(P>0.05);CT value of common carotid artery between dual energy CTA and single energy CTA was compared,the difference was not significant statistically(P>0.05);the CT value of internal jugular vein of dual energy CTA was higher than that of single energy CTA,and the noise of erector spinalis muscle and supraspinatus muscle were lower than that of single energy CTA,the differences were significant statistically(P<0.05);the image quality score of dual energy CTA was higher than that of single energy CTA,and the radiation dose was lower than that of single energy CTA,the differences were significant statistically(P<0.05).Conclusions Both dual energy CTA and single energy CTA are ideal in the diagnosis of carotid body tumor,which are consistent with the results of DSA,but compared with single energy CTA,dual energy CTA has smaller radiation dose,better image quality and more clinical application value.
