宫颈环形电切术治疗高级别CIN的效果及术后发生宫颈粘连的影响因素分析

Efficacy of circular cervical resection and influencing factors of postoperative cervical adhesion in patients with high grade CIN

目的探讨宫颈环形电切术治疗高级别宫颈上皮内瘤变(CIN)的效果及术后发生宫颈粘连的影响因素。方法抽取2017年2月至2019年2月济宁医学院附属湖西医院收治的CIN患者120例,其中CINⅡ级71例,CINⅢ级49例,均行宫颈环形电切术治疗,回顾性总结术后宫颈粘连发生情况,并应用Logistic分析其危险因素。结果120例CIN患者顺利完成手术,手术时间为(11.27±2.09)min,术中出血量为(15.29±2.83)ml,手术切口愈合时间为(22.91±6.74)d,术后发生宫颈粘连8例。宫颈粘连危险因素Logistic分析结果显示,年龄≥50岁OR=1.996、P=0.001,CINⅢ级OR=2.329、P=0.001,术后感染OR=1.743、P=0.001。结论宫颈环形电切术手术时间短,术中出血量少。年龄≥50岁、CINⅢ级、术后感染是宫颈环形电切术后发生宫颈粘连的危险因素。

Objective To investigate the application of circular cervical resection in high level cervical intraepithelial neoplasia(CIN)and the influencing factors of postoperative cervical adhesion.Methods A total of 120 CIN patients admitted to Huxi Hospital Affiliated to Jining Medical College from February 2017 to February 2019 were selected,including 71 cases of CIN grade II and 49 cases of CIN grade III,all of them underwent cervicectomy.Their postoperative cervical adhesions were reviewed retrospectively,and the risk factors were analyzed by Logistic.Results The 120 cases of CIN completed the operation successfully,the operation time was(11.27±2.09)min,the intraoperative blood loss was(15.29±2.83)ml,the operative wound healing time was(22.91±6.74)days,and 8 cases of cervical adhesions occurred after operation.The Logistic analysis of the risk factors of cervical adhesions showed that OR=1.996,P=0.001 for age≥50 years,OR=2.329,P=0.001 for CINⅢ,OR=1.743,P=0.001 for postoperative infection.Conclusions The operative time of cervix circular resection is short and the amount of blood loss is small.Age≥50 years,CIN gradeⅢ,and postoperative infection are the risk factors for cervical adhesions after cervical loop electrotomy.
