基于数据挖掘的弥漫型子宫腺肌病患者年龄分布及用药规律对比分析

Comparative Analysis of Age Distribution and Medication Patterns in Patients with Diffuse Adenomyosis Based on Data Mining

目的采用数据挖掘方法探析弥漫型子宫腺肌病的年龄分布及用药规律差异,以扩宽该病保守性治疗的思路。方法收集弥漫型子宫腺肌病患者资料535例,处方数564个,中药信息11002条,影像学资料780条,采用Liquorice软件进行复杂网络分析。结果发病年龄在40-49岁的患者最多,约占总患者人数的45.42%;弥漫型子宫腺肌病患者发病年龄在20-29岁的核心处方为:醋香附、醋延胡索、当归、川芎、白芍、蒲黄;30-39岁的核心处方为:醋香附、醋延胡索、当归、川芎、白芍、蒲黄、盐小茴香、益母草;40-49岁的核心处方为:醋香附、醋延胡索、当归、川芎、蒲黄、白芍、益母草、党参;50-55岁的核心处方为益母草、海螵蛸、党参、蒲黄、茜草、三七粉。结论40-49岁为弥漫型子宫腺肌病的好发年龄段,且发病年龄呈现年轻化态势。治疗弥漫型子宫腺肌病应在活血化瘀的基础上,根据年龄症状表现的不同,佐以理气止痛、益气养血之法。寒凝血瘀者散寒祛瘀,热毒瘀结者清热解毒化瘀,以血瘀出血证为主者则应化瘀止血。

Objective To explore the differences in age distribution and medication pattern of diffuse adenomyosis using data mining method,in order to broaden the ideas of conservative treatment of this disease.Methods 535 patients with diffuse adenomyosis,564 prescriptions,11,002 pieces of herbal information and 780 pieces of imaging data were collected and analyzed by complex network using Liquorice software.Results The age of onset was highest in patients aged 40-49 years,accounting for approximately 45.42%of the total number of patients.The core prescription for patients with diffuse adenomyosis aged 20-29 years old is vinegar processed Rhizoma Cyperi,vinegar processed Rhizoma Corydalis,Radix Angelicae Sinensis,Rhizoma Ligustici Chuanxiong,Radix Paeoniae Alba,Pollen Typhae;The core prescription for patients aged 30-39 is vinegar processed Rhizoma Cyperi,vinegar processed Rhizoma Corydalis,Radix Angelicae Sinensis,Rhizoma Ligustici Chuanxiong,Radix Paeoniae Alba,Pollen Typhae,salt fennel and motherwort;The core prescription for patients aged 40-49 is vinegar processed Rhizoma Cyperi,vinegar processed Rhizoma Corydalis,Radix Angelicae Sinensis,Rhizoma Ligustici Chuanxiong,Pollen Typhae,Radix Paeoniae Alba,motherwort and Radix Codonopsis;The core prescription for patients aged 50-55 is motherwort,cuttlebone,Radix Codonopsis,Pollen Typhae,Rubiae Radix et Rhizoma and Panax Notoginseng Powder.Conclusion The age of 40-49 years is the most common age group for diffuse adenomyosis,and the age of onset is becoming younger.The treatment of diffuse adenomyosis should be based on activating blood circulation and removing blood stasis,and depending on the age of the patient,the treatment should be complemented with qi to relieve pain and benefit qi to nourish blood.For those with cold stagnation and blood stasis,cold should be dispersed to remove stasis;for those with heat and toxin stasis,heat should be cleared to remove toxin and remove stasis;for those with blood stasis and bleeding,blood stasis should be removed to stop bleeding.
