免疫球蛋白及补体C3、C4检测在儿童原发性肾病综合征中的临床价值

The Clinical Value of Immunoglobulin and Complement C3,C4 Detection in Children with Primary Nephrotic Syndrome

目的讨论血清免疫球蛋白(IgG、IgA、IgM)以及血清补体(C3、C4)在儿童原发性肾病综合征临床诊疗中的应用价值。方法方便选取在该院接受诊治处理的儿童原发性肾病综合征患儿158例作为研究样本,接受诊治时间为2017年2月—2020年3月,将其作为观察组,另选取同期健康体检儿童150名作为参照组。予以全部入选儿童免疫球蛋白以及补体水平检测,检测方法采取免疫比浊法,记录、对比两组儿童的免疫球蛋白(IgG、IgA、IgM)以及补体(C3、C4)水平检测结果。结果观察组儿童的IgG水平较之参照组表现为明显的降低趋势[(6.32±1.51)g/L vs(8.24±2.25)g/L],差异有统计学意义(t=-8.834,P<0.05),而IgM水平较之参照组表现为明显的提升趋势[(2.84±0.58)g/L vs(2.23±0.43)g/L],差异有统计学意义(t=10.441,P<0.05)。结论免疫球蛋白以及补体在儿童原发性肾病综合征患儿临床诊断中的应用价值较为理想,在疾病诊断、预后评估中具有重要参考价值。

Objective To discuss the value of serum immunoglobulin(IgG,IgA,IgM)and serum complement(C3,C4)in the clinical diagnosis and treatment of primary nephrotic syndrome in children.Methods 158 children with primary nephrotic syndrome who were diagnosed and treated in the hospital were conveniently selected as the research sample.The diagnosis and treatment period were from February 2017 to March 2020.They were selected as the observation group and the same period health examination was selected.150 children served as the reference group.All selected children were tested for immunoglobulin and complement levels.The test method was immunoturbidimetric method.The test results of immunoglobulin(IgG,IgA,IgM)and complement(C3,C4)levels of the two groups of children were recorded and compared.Results The IgG level of children in the observation group showed a significant decrease compared with the reference group[(6.32±1.51)g/L vs(8.24±2.25)g/L],the difference was statistically significant(t=-8.834,P<0.05),and the level of IgM showed a significant increase compared with the reference group[(2.84±0.58)g/L vs(2.23±0.43)g/L],the difference was statistically significant(t=10.441,P<0.05).Conclusion The application value of immunoglobulin and complement in the clinical diagnosis of children with primary nephrotic syndrome is relatively ideal,and has important reference value in disease diagnosis and prognostic evaluation.
