基于数据挖掘分析围手术期耳穴运用的规律

Analysis of Ear Acupoint Application in Perioperative Period based on Data Mining

目的:通过数据挖掘研究围手术期耳穴运用的规律。方法:检索中国知网(CNKI)、重庆维普、万方数据库从建库至2019年12月31日关于围手术期耳穴治疗的文献330篇,运用"古今医案云平台V2.2.1"进行手术方式、围手术期症状、耳穴刺激方式、耳穴的频次分析,围手术期症状与耳穴、耳穴配伍的关联性分析。结果:围手术期27个症状,前5位是疼痛、焦虑、睡眠异常、胃肠功能异常、尿闭;所有处方共有71个耳穴,前10位是神门、皮质下、交感、胃、肾、大肠、脾、心、小肠、肝。围手术期核心耳穴处方:皮质下、神门、交感。针对围手术期常见的5个症状分别有核心耳穴处方:疼痛-神门、皮质下;焦虑-神门、内分泌、心、皮质下、肾、肝;睡眠异常-神门、皮质下、心、肾、脾、肝、交感;胃肠功能异常-交感、胃、大肠、小肠、脾;尿闭-膀胱、肾、尿道、三焦。结论:本研究的核心处方是皮质下、神门、交感,在此基础上根据临床症状、中医辨证、手术部位和经验加减配穴治疗。

Objective:To study the rules of ear acupuncture during perioperative period through data mining.Methods:Retrieved 330 documents on perioperative ear acupuncture treatment in the databases of CNKI,Chongqing Weipu and Wanfang from the establishment of the database to December 31,2019,and used"Ancient and Modern Medical Records Cloud Platform V2.2.1"The operation method,perioperative symptoms,ear acupoint stimulation methods,frequency analysis of ear acupoints,and correlation analysis of perioperative symptoms and the compatibility of ear acupoints and ear acupoints were carried out.Results:There were 27 symptoms in the perioperative period.The top 5 were pain,anxiety,abnormal sleep,abnormal gastrointestinal function,and urinary obstruction.There were 71 ear points in all prescriptions,and the top 10 were Shenmen,Subcortical,Sympathetic,Stomach,and Kidney,Large intestine,spleen,heart,small intestine,liver.Prescriptions for core ear points during perioperative period:subcortical,Shenmen,sympathetic.There are prescriptions for core ear acupoints for the 5 common symptoms during the perioperative period:Pain-Shenmen,Subcortical;Anxiety-Shenmen,Endocrine,Heart,Subcortical,Kidney,Liver;Sleep Disorders-Shenmen,Subcortical,Heart,Kidney,spleen,liver,sympathy;Abnormal gastrointestinal function-sympathy,stomach,large intestine,small intestine,spleen;urinary obstruction-bladder,kidney,urethra,triple burn.Conclusion:The core prescriptions of this study are subcortical,Shenmen,and sympathetic,based on the clinical symptoms,TCM syndrome differentiation,surgical site and experience to add and subtract acupoint treatment.
