摘要
目的分析血清胎盘生长因子(PLGF)、中性粒细胞与淋巴细胞比值(NLR)在子痫前期(PE)患者中的表达及临床意义。方法收集2019年6月1日~2020年10月1日定期产检并住院分娩的子痫前期患者71例为实验组,同期正常妊娠患者72例为对照组,收集以上患者的一般情况(年龄、孕产次、血压),分析两组患者PLGF、NLR、24h尿蛋白、终止妊娠孕龄、新生儿体重之间的相关性。结果 PLGF实验组明显低于对照组(P<0.01);而PLGF与血压、24h尿蛋白呈负相关,与终止妊娠孕龄、新生儿体重呈正相关;NLR实验组高于对照组(P<0.01);NLR与血压、24h尿蛋白呈正相关,与终止妊娠孕龄、新生儿体重呈负相关。根据ROC曲线,发现PLGF截断值为196ng/L(AUC=0.916,灵敏度88.7%,特异度77.8%),NLR截断值为3.57(AUC=0.839,灵敏度88.7%,特异度70.6%),NLR/PLGF截断值为72.146(AUC=0.913,灵敏度91.5%,特异度76.4%)。结论子痫前期患者PLGF低表达,NLR高表达,且与病情严重程度相关,二者均对诊断及预测子痫前期有较高意义,二者联合可提高诊断的灵敏度。
Objective Analysis of the expression and clinical significance of serum placental growth factor(placental growth factor, PLGF),neutral granulocyte-to-lymphocyte ratio(NLR) in pre-eclampsia(PE) patients.Methods Collected 71 cases of pre-eclampsia patients who had regular birth examination and hospital delivery were experimental groups, and 72 cases of normal pregnancy patients were in the control group during the same period, collecting the general situation of the above patients(age, motherhood, blood pressure),analyzing the correlation between PLGF,NLR,24 h urinary protein, termination of pregnancy and newborn weight in two groups of patients.Results The PLGF experimental group was significantly lower than the control group(P<0.01),while the PLGF was negatively associated with blood pressure and 24 h urinary protein, positively related to the termination of pregnancy and newborn weight, and the NLR experimental group was higher than the control group(P<0.01),and NLR was positively related to blood pressure and 24 h urine protein, and negatively related to termination of pregnancy and newborn weight.According to the ROC curve, the PLGF truncation value was found to be 196 ng/L(AUC=0.916,sensitivity 88.7%,specificity 77.8%),and NLR truncation value was 3.57(AUC=0.839,sensitivity 88.7%,specificity 70.6%),NLR/PLGF truncation value of 72.146(AUC=0.913,sensitivity 91.5%,specificity 76.4%).Conclusion The low expression of PLGF in patients with preeclampsia and the high expression of NLR are related to the severity of the disease.Both have high significance in the diagnosis and prediction of preeclampsia.The combination of the two can improve the sensitivity of diagnosis.
