摘要
目的研究日本2012年和2015年公布的肺龄评估公式是否适用于中国慢性阻塞性肺疾病(简称慢阻肺)人群,直观反映肺的损害程度.方法用日本肺龄评估公式计算健康组(160例)和慢阻肺组(102例)的肺龄.通过健康组评估肺龄(ELA)与生理肺龄(CLA)的差异和慢阻肺组?LA(?LA=ELA-CLA)的均值与参考值的差异来评估其适用性.结果在健康组中,2012年公式计算的ELA与CLA非常接近,两者的差异无统计学意义(P=0.187),而2015年公式的ELA明显低于CLA,两者的差异有统计学意义(P=0.000).在慢阻肺组中,2012年公式在中度、重度、极重度组中?LA的均值分别为7.8、16.3、31.0,除中度组外均明显高于参考值(14.2),各组间差异有统计学意义(P=0.000).2015年公式则分别为4.0、7.4、9.0,均低于参考值,组间无差异(P>0.05).结论日本2012年和2015年公布的肺龄评估公式在健康人群及慢阻肺人群中均有误差,但2012年公式误差更小,更能反映慢阻肺人群的肺损害程度.
Objective To study whether the lung age assessment formula published by Japan in 2012 and 2015 can be applied to the Chinese COPD population,representing the degree of lung damage.Methods The lung age of healthy group(160 total)and COPD group(102 total)were estimated using the lung age reference eqimtion published by Japan.For healthy group,assess the difference between the estimated lung age(ELA)and chronological lung age(CLA),the difference between the mean value of ALA(ALA=ELA~CLA)and the reference value for COPD group.Results In the healthy group,ELA in 2012 equation was very close to CLA,with no statistical significance(P=0.187),while ELA in 2015 equation was significantly lower than CLA,with statistical significance(P=0.000).In the COPD group,the mean values of ALA in the moderate,severe and extremely severe group in 2012 equation were respectively 7.8,16.3 and 31.0,among which,the mean values of ALA in the severe and extremely severe groups were sigiiifR antly higher than the reference values,and the difference between each group was statistically significant(P=0.000).The mean values of ALA in different groups in 2015 equation were 4.0,7.4and 9.0.Each group was lower than the reference value,and there was no difference between groups(P>0.05).Conclusion The lung age nssessmenr fonnuhis published in Japan in 2012 and 2015 have errors in healthy people and COPD people,hut the fonmib in 2012 has smaller errors and can better reflect the degree of lung damage in COPD people.
