LOXL2蛋白在食管鳞癌中的表达及临床病理学意义 被引量:1

摘要 目的探讨食管鳞癌组织中LOXL2的表达状态与临床病理学特征之间的关系并评估预后风险。方法采用免疫组织化学染色检测106例Ⅰ-Ⅲ期食管鳞癌组织和癌旁组织(距癌组织>5 cm)以及42例Ⅳ期食管鳞癌癌组织中LOXL2蛋白的表达水平,分析LOXL2表达与临床病理特征及预后的关系。结果食管鳞癌组织和癌旁组织中LOXL2蛋白阳性表达率分别为66.98%和37.73%(χ^(2)=18.173,P<0.01),Ⅰ期、Ⅱ期、Ⅲ期和Ⅳ期食管鳞癌患者中LOXL2蛋白阳性表达率为30.00%、69.44%、71.67%和92.86%;LOXL2阳性表达与肿瘤的分化程度、浸润程度、淋巴结转移和临床分期差异均有统计学意义(χ^(2)=15.468、15.839、6.332、18.526,P均<0.05),与患者的年龄、民族、性别和癌胚抗原表达差异均无统计学意义(χ^(2)=0.63、6.094、0.182、2.992,P均>0.05)。LOXL2阳性表达患者总生存率及总生存时间显著缩短,LOXL2阳性表达组患者的总生存率为20.90%,LOXL2阴性表达组总生存率为44.74%(χ^(2)=14.32,P<0.01)。Cox多因素分析显示,临床分期和LOXL2阳性是食管鳞癌的独立预后危险因素(HR=2.99,P<0.01;HR=1.73,P<0.05)。结论LOXL2蛋白在食管鳞癌组织中阳性表达率高,LOXL2阳性表达与肿瘤分化程度、浸润深度、淋巴结转移和TNM分期相关,LOXL2阳性表达的食管鳞癌患者预后不良。 Objective To explore the relationship between LOXL2 expression status and clinicopathological features in patients with esophageal squamous cell carcinoma(ESCC)and assess the risk of prognosis.Methods Immunohistochemical staining was used to detect the expression of LOXL2 protein in cancerous tissues and adjacent tissues of 106 patients with Ⅰ-Ⅲ ESCC and 42 patients with IV cancer,analysis of the relationship between LOXL2 expression and clinicopathological features and prognosis.Results The expression of LOXL2 protein positive expression rates in the ESCC and in the paracancer group were 66.98%and 37.73%(χ^(2)=18.173,P<0.01).LOXL2 expression rates were 30.00%,69.44%,71.67%and 92.86%respectively in patients with ESCC at Ⅰ,Ⅱ,Ⅲ and Ⅳ stages.LOXL2 positive expression was positively correlated with tumor differentiation,tumor invasion,lymph node metastasis and clinical stage(χ^(2)=15.468,15.839,6.332 and 18.526,P<0.05);there was no significant difference in the expression of carcinoembryonic antigen with age,ethnicity,sex of patients.Survival analysis found that survival rate and survival time of LOXL2 positive expression patients significantly shortened,LOXL2 overall survival rate was 20.90%in the positive expression group and 44.74%in the negative expression group(χ^(2)=14.32,P<0.01).COX multivariate analysis showed that LOXL2 and clinical staging were independent prognostic risk factors for ESCC(HR=2.99,P<0.01;HR=1.73,P<0.05).Conclusion This study confirms that LOXL2 positive expression rate is higher in ESCC.Positive expression of LOXL2 is associated with differentiation,depth of invasion,lymph node metastasis and TNM staging of ESCC.And the prognosis of LOXL2 positive expression of ESCC is poor.
