30例主动脉夹层患者认知功能与血清C反应蛋白水平的相关性分析

Correlation analysis between cognitive function and serum C-reactive protein level in 30 patients with aortic dissection

目的探讨主动脉夹层(AD)患者认知功能与C反应蛋白(CRP)水平的相关性。方法使用病例对照研究方法,选择新疆医科大学第一附属医院B型主动脉夹层腔内隔绝术后6~12个月并于门诊随访的30例患者(AD组)进行研究,同期筛选30例性别和年龄匹配的高血压(HTN)患者作为对照组(HTN组)。使用可重复性神经心理测查系统(RBANS)评估认知功能,使用全自动生化分析仪测定血CRP水平。比较两组患者的RBANS评分以及CRP水平,并使用皮尔逊相关分析探索认知功能与CRP的相关性。结果最终纳入60例患者进行研究。AD组CRP水平明显高于HTN组(P<0.001)。AD组认知功能明显较HTN组差,尤其表现在即刻记忆、视觉广度、言语功能、注意力、延迟记忆以及测验总分(P<0.05)。通过Pearson相关分析得出,CRP与BRANS中的测试总分(r=0.404,P=0.027)、即刻记忆(r=0.379,P=0.039)、延迟记忆(r=0.476,P=0.008)、词汇学习(r=0.391,P=0.033)具有明显的相关性。结论在AD患者中,CRP与认知功能可能呈正相关关系。对AD患者进行CRP水平检测,可能有助于评估患者的认知功能。

Objective To explore the correlation between cognitive function and serum C-reactive protein(CRP)level in patients with aortic dissection(AD).Methods In this case control study 30 patients with aortic dissection who followed-up after endovascular isolation and 30 gender and age-matched patients with hypertension(HTN)were recruited from the First Affiliated Hospital of Xinjiang Medical University.Cognition function was assessed utilizing the Repeatable Battery for the Assessment of Neuropsychological Status(RBANS).And blood CRP levels were quantified using an automatic biochemistry analyzer.RBANS score and CRP levels were compared between the two groups,and Pearson correlation analysis was used to explore the correlation between cognitive function and CRP.Results Sixty patients were eventually enrolled in the study.We compared gender,age,smoking history,education level and other factors between the two groups,and no significant differences were found between the two groups.The levels of CRP in AD group was significantly higher than that in HTN group(P<0.001).The cognitive function of AD group was significantly worse than that of HTN group,especially in immediate memory,visuospatial/constructional ability,language,attention,delayed memory and test total score(P<0.05).Pearson correlation analysis showed that CRP was significantly correlated with RBANS total test score(r=0.404,P=0.027),immediate memory(r=0.379,P=0.039),delayed memory(r=0.476,P=0.008),and vocabulary learning(r=0.391,P=0.033).Conclusion In patients with AD,CRP may be positively correlated with cognitive function.Detection of CRP levels in patients with AD may be helpful in assessing cognitive function.
