自体肋软骨“瓦合支架法”在鼻综合整形术中的应用效果研究

Study on the Application Effect of Autologous Costal Cartilage“Tiling Stent Method”in Comprehensive Rhinoplasty

目的:研究自体肋软骨“瓦合支架法”在鼻综合整形术中的临床应用效果。方法:纳入158例实行鼻综合整形术的就医者,选取右侧第7肋内侧部分肋软骨作为鼻中隔延伸移植物(Septal extensional graft,SEG)、鼻小柱支撑移植物(The nasal columella support graft,STRUT),按照“瓦合支架法”塑造鼻形,收集术前、术后鼻部照片,测量鼻部形态数据,行统计学处理。结果:153例就医者术后愈合良好,无鼻部形变,歪曲,鼻形态明显改善;5例出现切口感染。鼻宽/鼻高、鼻根宽/鼻高及鼻根高/鼻高术前术后比较差异无统计学意义(P>0.05);鼻尖高/鼻高及鼻背长/鼻高较术前有所改善,差异有统计学意义(P<0.05)。就医者满意度为94%。结论:自体肋软骨“瓦合支架法”在鼻综合整形术中的应用对于抬高鼻尖、延长鼻背、改善鼻部整体美学形态及减少术后并发症均有明确的改善效果。

Objective Study the clinical application of autologous costal cartilage“tiling stent method”in comprehensive rhinoplasty.Methods A total of 158 cases of beauty seekers undergoing comprehensive rhinoplasty were included.The right 7th medial costal cartilage was selected as the nasal septal extensional graft(SEG),and nasal column support graft(STRUT).The nasal morphology was modeled according to the“tiling stent method”,and the nasal photos of the preoperative and postoperative view were collected,and the nasal morphology data was measured and statistically processed.Results The wound healed well in 153 patients,without nasal deformation and distortion.Their nasal morphology has been improved.But 5 cases had incision infection.There was no significant difference in nasal width/height,nasal root width/height and nasal root height/height before and after operation(P>0.05).The nasal tip height/nasal height and nasal back length/nasal height were improved compared with those before operation,and the differences were statistically significant(P<0.05).The satisfaction of the patients was 94%.Conclusion The application of autologous costal cartilage“tiling stent method”in rhinoplasty has clear effects on raising the tip of the nose,prolonging the nasal dorsum,improving the overall aesthetic shape of the nose and reducing postoperative complications.
