调督整脊理论治疗腰椎间盘突出症的临床观察

Clinical observation on the treatment of lumbar disc herniation with the theory of regulating governor channel and setting spine

目的探讨运用调督整脊理论治疗腰椎间盘突出症的临床观察。方法将80例确诊为腰椎间盘突出症的患者随机分为2组,每组各40例。观察组依据调督法理论,采用手法正骨联合腰部夹脊穴火针疗法进行治疗;对照组运用艾瑞西布片(江苏恒瑞医药股份有限公司)配合甲钴胺分散片(江苏四环生物制药有限公司)治疗。治疗后对比2组的总有效率及视觉模拟评分法(VAS)。结果观察组治愈26例,显效8例,好转4例,无效2例,总有效率为95%;对照组治愈16例,显效6例,好转10例,无效8例,总有效率80%。2组比较,差异有显著统计学意义(P<0.05)。在VAS评分方面,治疗1个疗程后即有差异,但此时并无显著差异(P>0.05),在治疗3个疗程后差异明显,观察组评分显著低于对照组(P<0.01)。结论依据调督整脊理论,采用手法正骨配合腰部夹脊穴火针疗法,可以恢复腰椎骨-肌生物力学平衡,增强脊柱稳定性,改善神经根与突出物位置关系,能有效改善患者腰椎间盘突出症,治疗腰椎间盘突出症疗效确切,无不良反应发生,安全性、普适性强,适合临床广泛推广。

Objective To explore the clinical observation on the treatment of lumbar disc herniation with the theory of regulating government channel and setting spine.Methods 80 patients diagnosed with lumbar disc herniation were randomly divided into two groups,with 40 cases in each group.The observation group was treated with manual bone setting manipulation combined with fire needle therapy at Jiaji points on the waist based on the theory of governor channel regulating method.The control group was treated with Imrecoxib Tablets(Jiangsu Hengrui Pharmaceutical Co.,Ltd.)combined with mecobalamin dispersible tablets(Jiangsu Sihuan Biopharmaceutical Co.,Ltd.).After treatment,the total effective rate and visual analogue scale(VAS)of the two groups were compared.Results In the observation group,26 cases were cured,8 cases were markedly effective,4 cases were improved,and 2 cases were ineffective,and the total effective rate was 95%.In the control group,16 cases were cured,6 cases were markedly effective,10 cases were improved,and 8 cases were ineffective,and the total effective rate was 80%.The difference between the two groups was statistically significant(P<0.05).In terms of VAS score,a difference was observed after only one course of treatment,yet the difference at this time was not significant(P>0.05).The difference turned obvious after three courses of treatment with the score of the observation group significantly lower than that of the control group(P<0.01).Conclusion According to the theory of regulating governor channel and setting spine,bone-setting manipulation combined with fire needle therapy at the lumbar Jiaji points can restore the biomechanical balance of the lumbar spine and muscles,enhance the stability of the spine,improve the positional relationship between nerve roots and protrusions,and can effectively improve the patients’symptoms.The treatment is characterised as a definite curative effect,no adverse reactions,higher safety and universality,therefore it is suitable for widespread clinical promotion.
