双穿刺点对比单穿刺点腰硬联合麻醉在剖腹产术中及术后镇痛优势对比

Advantages Comparison of Dual Puncture Points Versus Single Puncture Point Combined Spinal-epidural Anesthesia During And After Cesarean Section

目的探讨双穿刺点对比单穿刺点腰硬联合麻醉在剖腹产术中及术后镇痛效果差异。方法选取2018年03月至2020年03月我院实施剖腹产手术产妇60例进行研究,用随机分组表将产妇分为双穿刺组(双穿刺点腰硬联合麻醉,30例)和单穿刺组(单穿刺点腰硬联合麻醉,30例),比较两组产妇剖腹产术中、术后12h疼痛情况及相关麻醉不良反应发生情况之间的差异。结果双穿刺组产妇术中15min、术后12h疼痛评分均显著低于单穿刺组([2.33±0.42 vs.3.21±0.54,P<0.001],[3.53±0.62 vs.4.18±0.56,P<0.001])。围手术期期间,两组麻醉呼吸抑制、低血压、恶心呕吐等相关不良反应发生率无显著差异([0.00% vs.0.00%,P=1.000],[6.67% vs.10.00%,P=0.640],[13.33% vs.6.67%,P=0.389])。结论在剖腹产产妇中实施双穿刺点下腰硬联合麻醉,相对单穿刺点下腰硬联合麻醉,在术中、术后具有更显著镇痛效果,安全性较高,值得推广应用。

Objective to investigate effect of dual puncture points versus single puncture point combined spinal-epidural anesthesia during and after cesarean section.Methods We chose 60 cases women with caesarean section in our hospital from March 2018 to March 2020 for research.Divided them into double puncture group (double puncture point combined spinal-epidural anesthesia,30 cases) and single puncture group using a randomized grouping table (single puncture point combined spinal-epidural anesthesia,30 cases).Compared differences in pain and related adverse reactions of anesthesia during and 12 hours after caesarean section between two groups.Results maternal pain scores during 15 minutes and 12 hours after double puncture group were significantly lower than single puncture group ([2.33±0.42 vs.3.21±0.54,P<0.001],[3.53±0.62 vs.4.18±0.56,P<0.001]).During perioperative period,there was no significant difference in incidence of related adverse reactions such as respiratory depression,hypotension,nausea and vomiting between two groups ([0.00% vs.0.00%,P=1.000],[6.67% vs.10.00%,P=0.640],[13.33% vs.6.67%,P=0.389]).Conclusion dual puncture point combined spinal-epidural anesthesia is performed in cesarean parturient women.Compared with single puncture point combined spinal-epidural anesthesia,it has more significant analgesic effect during and after operation,which is safe and worthy of promotion.
