MRC肌功能矫治器干预治疗儿童安氏Ⅱ类Ⅰ分类错颌畸形的临床应用研究 被引量:19

摘要 目的探讨MRC肌功能矫治器干预治疗儿童安氏Ⅱ类Ⅰ分类错颌畸形的临床应用效果。方法回顾性分析2016年7月至2019年8月上海沪东医院收治的80例替牙期安氏Ⅱ类Ⅰ分类错颌畸形患儿的临床资料,其中45例患儿采用合适的软质MRC肌功能矫治器进行错颌畸形矫治,设为研究组;35例患儿采用传统的功能性矫治器进行错颌畸形矫治,设为对照组。两组矫治疗程均为9~15个月。统计并比较两组患儿矫治结束后的口腔不良习惯改善情况、矫治效果主观评价情况,以及矫治前后的头影测量情况。结果研究组口腔矫治不良习惯改善的总有效率高于对照组[86.67%(39/45)比68.57%(24/35)](P<0.05)。研究组佩戴顺利、佩戴舒适性、佩戴依从性评分均显著高于对照组[(4.13±0.28)分比(3.56±0.24)分、(3.85±0.27)分比(3.12±0.28)分、(3.54±0.23)分比(3.22±0.21)分](P<0.01)。研究组治疗后蝶鞍点-鼻根点-下齿槽座点角(SNB)、下中切牙长轴与鼻根点-上齿槽座点连线的垂直距离(L1-NB)、下颌平面角(FH-MP)较治疗前和对照组治疗后显著增加(P<0.05),上下齿槽座角(ANB)、上中切牙长轴与鼻根点-上齿槽座点连线的垂直距离及交角(U1-NA)较治疗前和对照组治疗后显著降低(P<0.05)。结论与传统的功能性矫治器相比,MRC肌功能矫治器在安氏Ⅱ类Ⅰ分类错颌畸形矫治方面的效果更为显著,能够有效地纠正患儿口腔不良习惯,患儿接受度较高。 Objective To explore the clinical application of MRC orthopedic device in the treatment of children with angle class Ⅱ division Ⅰ malocclusion.Methods A total of 80 children with angle class Ⅱ division Ⅰ malocclusion in the stage of dental replacement admitted to Shanghai Hudong Hospital from Jul.2016 to Aug.2019 were retrospectively analyzed,including 45 patients treated with appropriate soft MRC muscle functional orthopedic device assigned to a study group,and 35 patients treated with traditional functional orthopedic device for malocclusion as a control group.Both groups were treated for 9-15 months.The improvement of oral bad habits,the subjective evaluation of the effect of correction,and the cephalometry before and after the correction were statistically recorded and compared between the two groups.Results The effective rate of improvement of bad oral habits in the study group was significantly higher than that in the control group[86.67%(39/45)vs 68.57%(24/35)](P<0.05).The scores of smooth wearing,comfortable wearing and compliance of the study group were significantly higher than those of the control group[(4.13±0.28)vs(3.56±0.24),(3.85±0.27)vs(3.12±0.28),(3.54±0.23)vs(3.22±0.21)](P<0.01).After treatment,sella-nasion-B point(SNB),distance from L1 to NB(L1-NB)and FH-MP in the study group were significantly increased than before treatment and the control group after treatment(P<0.05),AB plane angle(ANB),distance from U1 to NA and angle(U1-NA)were significantly decreased than before treatment and the control group after treatment(P<0.05).Conclusion MRC has a significant corrective effect in the treatment of Angle Class Ⅱ division Ⅰ malocclusion,which can effectively correct the bad oral habits of the children with high acceptance.
