肥胖型支气管哮喘患儿血Clara细胞分泌蛋白及补体C3和C4水平与病情严重程度的相关性分析 被引量:6

摘要 目的探讨肥胖(OB)型支气管哮喘患儿血Clara细胞分泌蛋白(CC16)、补体C3和C4与患儿病情严重程度以及肺功能之间的相关性。方法选取2016年8月至2019年8月收治的315例支气管哮喘患儿,依据患儿OB情况分为OB组(n=169)和非OB组(n=146),比较两组患儿血CC16、补体C3以及C4水平,随后依据OB组患儿肺功能分度以及病情严重程度情况分为4组,比较OB组患儿不同肺功能分度、病情严重程度患儿血CC16、补体C3、C4水平,并分析血CC16、补体C3、C4水平与肺功能分度、病情严重程度之间相关性。结果OB组患儿血CC16水平显著低于非OB组,补体C3以及C4水平显著高于非OB组(P<0.05);Pearson相关性分析显示,CC16与补体C3、补体C4负相关(r=-0.694,-0.603;P<0.05),而补体C3与补体C4正相关(r=0.512;P<0.05);不同病情严重程度患儿上述指标之间比较差异有统计学意义(P<0.05);Spearson相关性分析结果显示,CC16水平与肺功能分度、病情严重程度负相关(r=-0.629,-0.576;P<0.05),补体C3以及C4水平与病情严重程度正相关(r=0.489,0.463;P<0.05),与肺功能分度正相关(r=0.542,0.479;P<0.05)。结论OB型支气管哮喘患儿血CC16异常上升,补体C3以及C4水平异常下降,患儿病情严重程度、肺功能异常与CC16、补体C3和C4水平异常关系密切。 Objective To investigate the associationsof blood Clara cell secretion protein(CC16)and complements C3 and C4 levels with disease severity and lung function in children with obese(OB)bronchial asthma.Methods We selected 315 children with bronchial asthma from August 2016 to August 2019 as research objects.They were divided into OB group(n=169)and non-OB group(n=146)according to the situation of obesity.The blood CC16 and complements C3 and C4 levels were compared between the two groups.Then,children in the OB group were divided into 4 subgroups according to the grade of lung function and the disease severity.The blood CC16 and complements C3 and C4 levels in children with different grades of lung function and different severities in the OB group were compared.The associations ofblood CC16 and complements C3 and C4 levels with grades of lung function and disease severity was analyzed.Results The blood CC16 levels in the OB group were significantly lower than those in the non-OB group while the complements C3 and C4 levels were higher than those in the non-OB group(P<0.05).Pearson correlation analysis showed that CC16 was negatively correlated with complement C3 and C4(r=-0.694 and-0.603,respectively,P<0.05),and complement C3 was positively correlated with complement C4(r=0.512,P<0.05).The results of Spearman correlation analysis showed that the CC16 level was negatively correlated with the grade of lung function and the severity(r=-0.629 and-0.576,respectively,P<0.05).Complements C3 and C4 levels were positively correlated with the severity(r=0.489 and 0.463,respectively,P<0.05),and positively correlated with the grade of lung function(r=0.542 and 0.479,respectively,P<0.05).Conclusion Children with OB bronchial asthma have abnormally elevated blood CC16 levels and abnormally decreased complements C3 and C4 levels.The severity of the disease and abnormal lung function are closely related to abnormal CC16,complements C3 and C4 levels.
