妊娠期良性阵发性位置性眩晕8例临床诊疗分析

Clinical analysis of benign paroxysmal positional vertigo during pregnancyin8 cases

目的分析妊娠期良性阵发性位置性眩晕患者的一般资料及临床特征,提高对该类人群BPPV的认识及诊疗水平。方法回顾性分析2019年5月~2020年2月诊治的8例妊娠期良性阵发性位置性眩晕患者临床资料,总结临床诊疗经验。8例患者均为妊娠期患者,年龄21-35岁,中位年龄为24岁,平均年龄28.1岁。孕周为11周-33周,中位孕周为25周,平均孕周为24周。其中首次妊娠3例,第二次妊娠5例;双胎妊娠1例,为试管婴儿,单胎7例。结果 8例患者均表现为相对于重力方向头位改变时,出现不同程度的眩晕,并伴有眼震,其中4例为后半规管BPPV,4例为外半规管(水平半规管)BPPV,均可行手法复位。结论在复位过程中,采用了慢速、温和和多人协作的复位方法,亦取得了良好的效果,因此手法复位同样适合妊娠期BPPV这类特殊患者,均能取得良好效果。

Objective To analyze the general data and clinical features of benign paroxysmal positional vertigo (BPPV) during pregnancy,so as to improve the understanding of BPPV and the level of diagnosis and treatment.Methods The clinical data of 8 patients with benign paroxysmal positional vertigo during pregnancy from May 2019 to February 2020 were retrospectively analyzed,and the clinical experience was summarized.All the 8 patients were pregnant patients,aged 21-35 years,with a median age of 24 years and an average age of 28.1 years.The gestational weeks ranged from 11 to 33 weeks,with a median of 25 weeks and an average of 24 weeks.There were 3 cases of first pregnancy,5 cases of second pregnancy,1 case of twin pregnancy,7 cases of single pregnancy.Results The 8 patients presented with vertigo and nystagmus when the head position was changed relative to the gravity direction.Among them,4 cases were posterior semicircular canal BPPV,and 4 cases were lateral semicircular canal (horizontal semicircular canal) BPPV.Conclusion In the process of reduction,the use of slow,gentle and multi person cooperation reduction method,also achieved good results,so manual reduction is also suitable for pregnant BPPV such as special patients,can achieve good results.
