右美托咪定在耳聋小儿人工耳蜗植入术麻醉中的应用

Application of Dexmedetomidine in Anesthesia of Cochlear Implantation in Deaf Children

目的评价右美托咪定在耳聋小儿人工耳蜗植入术麻醉中的应用价值。方法选取2019年1月~2020年8月来我院耳鼻咽喉科就诊的行人工耳蜗植入术治疗的耳聋患儿86例依据随机数表法分成两组,术中两组患儿均行丙泊酚、瑞芬太尼联合七氟烷静吸复合麻醉,对照组使用生理盐水,观察组使用右美托咪定麻醉,对比两组患儿不同时间段的血流动力学指标以及用药后的麻醉效果。结果用药前的心率和平均动脉压对比无显著差异,用药后5~30min观察组患儿的心率和平均动脉压指标与对照组相比,差异有统计意义(P<0.05);观察组患儿的拔管时间、苏醒时间以及躁动发生率均明显优于对照组,组间差异P<0.05。结论给予耳聋患儿行人工耳蜗植入术治疗中选择右美托咪定的麻醉效果更稳定,安全性高,值得临床推广采纳。

Objective To evaluate the application value of dexmedetomidine in anesthesia for cochlear implantation in deaf children.Method A selection of 86 children with deafness who underwent cochlear implantation treatment who came to our hospital from January 2019 to August 2020 for treatment with cochlear implantation were divided into two groups according to the random number table method.During the operation,both groups of children were treated with propionate Phenol,remifentanil combined with sevoflurane intravenous inhalation combined anesthesia,the control group was treated with normal saline,and the observation group was treated with dexmedetomidine.The hemodynamic indexes of the two groups of children at different time periods and the anesthesia after treatment were compared effect.Results There was no significant difference between the heart rate and the mean arterial pressure before the medication.The heart rate and mean arterial pressure of the observation group were statistically significant (P<0.05) compared with the control group 5-30 min after medication.The extubation time,wake-up time and the incidence of restlessness of the children were significantly better than those of the control group.The difference between the groups was P<0.05.Conclusion The anesthesia effect of dexmedetomidine is more stable and safe for cochlear implantation in deaf children,and it is worthy of clinical promotion and adoption.
