脑卒中患者创伤后成长的现状及影响因素分析

Analysis of the status quo and influencing factors of post-traumatic growth among stroke patients

目的探讨脑卒中患者创伤后成长的现状及影响因素的分析。方法选择2017年8月—2019年8月本院收治的210例脑卒中患者作为研究对象,对所有患者的社会支持、创伤后成长和一般资料进行问卷调查。统计患者的社会支持评分、创伤后成长评分,分析脑卒中患者创伤后成长的单因素,多重线性回归分析脑卒中患者创伤后成长因素的相关性。结果本次研究中,210例患者社会支持评分中,社会支持总分为(57.3±5.8)分;创生后成长总分为(58.2±12.6)分。以创伤后成长总分为观察指标,脑卒中患者创伤后成长的单因素包含性格类型、自觉痛苦程度、后遗症状、病程、诊断类型、经济收入和年龄,差异均有统计学意义(P<0.05)。多重线性回归分析显示,内向型性格、年龄水平为负相关性,社会支持、自觉痛苦程度、病程为正相关性,差异均有统计学意义(P<0.05)。结论脑卒中患者患病后一定程度都会发生创伤后成长,其创伤后成长水平受疾病因素和社会学因素影响,创伤后成长与患者的社会支持呈正相关。因此,脑卒中患者的护理中应给予个体化的心理支持措施,重视患者正性改变,关注患者负性心理感受,引导、帮助患者进行积极的心理调适,加速患者康复。

Objective To investigate the current status of post-traumatic growth among stroke patients and analyse influencing factors.Methods A total of 210 stroke patients those admitted to our hospital from August 2017 to August 2019 were selected as study subjects.A questionnaire survey was conducted on all patients on social support,post-traumatic growth,and general information.Statistical analysis was performed on patients'social support scores and post-traumatic growth scores,analysis of single factors of post-traumatic growth of stroke patients was performed,multiple linear regression analysis was applied to analyze the correlation of post-traumatic growth factors of stroke patients.Results In this study,among the 210 patients,the total score of social support was 57.3±5.8 points;the average total score of post-traumatic growth was 58.2±12.6 points.The total score of post-traumatic growth was set as an observation index,the single factors that affected post-traumatic growth of stroke patients were including personality type,degree of perceived pain,sequelae symptoms,disease course,diagnosis type,economic income,and age,all of which are statistically significant(P<0.05).Multiple linear regression analysis showed that introverted personality and age level were negatively correlated to post-traumatic growth,and social support,the degree of conscious pain,and the course of the disease are positively correlated to post-traumatic growth.Conclusions Post-traumatic growth occurs to a certain extent in stroke patients.Their post-traumatic growth level is affected by the patient's disease factors and sociology factors.Post-traumatic growth is positively correlated with the patient's social support.Therefore,individualized psychological support measures should be given in the nursing of stroke patients,pay attention to the positive changes of the patients,pay attention to the negative psychological feelings of the patients,guide and help the patients to make positive psychological adjustments,and accelerate the recovery of the patients.
