颅脑MRI联合新生儿振幅整合脑电图在高危儿脑损伤筛查中的应用 被引量:1

摘要 目的探究颅脑磁共振成像(MRI)联合新生儿振幅整合脑电图在高危儿脑损伤筛查中的应用价值。方法100例高危脑损伤患儿,根据检查方式的不同分为对照组(52例)及观察组(48例)。对照组采用颅脑MRI检测,观察组采用颅脑MRI联合新生儿振幅整合脑电图检测。比较两组疾病检出率,观察组中不同脑损伤程度缺氧缺血性脑病(HIE)患儿及不同等级颅内出血患儿的新生儿振幅整合脑电图指标。结果观察组疾病总检出率100.00%高于对照组的77.27%,差异有统计学意义(P<0.05)。观察组重度异常HIE患儿的睡眠-觉醒周期不成熟型占比、最高电压高于轻度异常及中度异常HIE患儿,最低电压低于轻度异常及中度异常HIE患儿,差异有统计学意义(P<0.05)。观察组颅内出血Ⅲ~Ⅳ级患儿的睡眠-觉醒周期不成熟型占比100.00%及最高电压(56.56±4.31)μV显著高于Ⅰ~Ⅱ级患儿的33.33%、(41.03±3.39)μV,最低电压(4.11±0.39)μV显著低于Ⅰ~Ⅱ级患儿的(6.99±0.60)μV,差异有统计学意义(P<0.05)。结论颅脑MRI联合新生儿振幅整合脑电图在高危儿脑损伤筛查中的应用价值较高,对于诊断脑损伤严重程度也有一定的意义。 Objective To investigate the practical value of brain magnetic resonance imaging(MRI)combined with neonate amplitude-integrated electroencephalogram in screening brain injury of high risk newborns.Methods A total of 100 high risk newborns were divided into control group(52 cases)and observation group(48 cases)according to different examination methods.The control group was examined with brain MRI,and the observation group was examined with brain MRI and neonate amplitude-integrated electroencephalogram.The disease detection rate was compared between the two groups,and the neonatal amplitude integrated electroencephalogram indexes of hypoxic-ischemic encephalopathy(HIE)neonates with different degrees of brain injury and neonates with different grades of intracranial hemorrhage were compared in the observation group.Results The total disease detection rate 100.00%of the observation group was higher than 77.27%of the control group,and the difference was statistically significant(P<0.05).In the observation group,the proportion of immature sleep wake cycle and highest voltage of HIE neonates with severe abnormalities were higher than those of HIE neonates with mild and moderate abnormalities,and the lowest voltage was lower than that of neonates with mild and moderate abnormalities.All the difference was statistically significant(P<0.05).In the observation group,the proportion of immature sleep wake cycle 100.00%and highest voltage(56.56±4.31)μV of neonates with intracranial hemorrhage gradeⅢ-Ⅳwere significantly higher than 33.33%and(41.03±3.39)μV of neonates with intracranial hemorrhage gradeⅢ-Ⅳ,and lowest voltage(4.11±0.39)μV was significantly lower than(6.99±0.60)μV of neonates with intracranial hemorrhage gradeⅠ-Ⅱ.All the difference was statistically significant(P<0.05).Conclusion Combination of brain MRI and neonate amplitude-integrated electroencephalogram shows high practical value in screening brain injury of high risk newborns,and it also has certain significance for diagnosis of the severity of brain injury.
