中性粒细胞与淋巴细胞比值对宫颈癌切除术后复发的预测价值

Prognostic value of neutrophil to lymphocyte ratio in recurrence of cervical cancer after resection

目的探讨血清中性粒细胞与淋巴细胞比值(NLR)对宫颈癌患者术后复发的预测价值。方法选取2013年1月至2019年1月在丽水市中心医院行宫颈癌切除术的307例患者作为研究对象。记录患者基线资料、临床资料以及术后随访资料,评估不同NLR水平患者之间的预后差异,并分析相关性。结果单因素分析显示术前外周血NLR水平与边缘浸润阳性、白细胞(WBC)计数较高、血小板-淋巴细胞比率(PLR)升高、绝对淋巴细胞计数(ALC)减低、绝对中性粒细胞计数(ANC)升高呈正相关(P<0.05)。高NLR组的复发率显著高于低NLR组(P<0.05)。多因素Logistic回归模型分析进一步证实,NLR升高、术中切除组织过少、边缘浸润阳性是宫颈癌患者术后复发的独立危险因素。结论宫颈癌患者NLR升高提示患者术后预后不良,密切监测NLR水平对宫颈癌术后患者的预后有重要的预测价值。

Objective To investigate the prognostic value of neutrophil to lymphocyte ratio(NLR)in recurrence of cervical cancer after resection.Methods 307 patients underwent cervical resection in Lishui Central Hospital from January 2013 to January 2019 were selected as the study subjects.Baseline data,clinical data and postoperative follow-up data were recorded to evaluate the prognostic differences among patients with different NLR levels and analyze the correlation.Results Univariate analysis showed that preoperative peripheral blood NLR level was positively correlated with positive marginal infiltrates,higher WBC count,increased platelet-lymphocyte ratio(PLR),decreased absolute lymphocyte count(ALC),and increased absolute neutrophils count(ANC).The recurrence rate in the high NLR group was significantly higher than that in the low NLR group.Multivariate Logistic regression model analysis further confirmed that increased NLR,less intraoperative excised tissue and positive edge infiltration were independent risk factors for postoperative recurrence of cervical cancer patients.Conclusions Increased NLR in patients with cervical cancer suggests poor postoperative prognosis,and close monitoring of NLR level has important predictive value for prognosis of patients with cervical cancer.
