应用MSCT诊断胸壁结核及临床手术指导价值分析

Application of MSCT in the Diagnosis of Chest Wall Tuberculosis and Analysis on Clinical Surgical Guidance Value

目的分析应用MSCT诊断胸壁结核及临床手术指导价值。方法回顾性分析我院2015年1月至2018年6月收治的99例胸壁结核患者的临床资料,观察MSCT检查所得结果进行诊断分析及术后治疗效果。结果 99例患者中,MSCT检查发现胸壁肿块81例,占81.82%;软骨、骨破坏65例,占65.66%;病灶内钙化43例,占43.43%;"蟹足样"窦道37例,占37.37%;肋骨破坏35例,占35.35%;病变位于胸壁软组织者51例;脓腔出现在肋间以及肌里,并且中间有窦道相通呈哑铃型病变者37例;43例患者液化区周围以及脓腔壁出现钙化。所有患者术后伤口均一期愈合,且随访3年无1例复发。结论 MSCT检查可清楚显示胸壁结核的影像学特点,根据相关组织病变之间的关系,可为临床上手术治疗提供正确的指导,有效改善患者生活质量,值得推广。

Objective To analyze and apply MSCT for diagnosis of chest wall tuberculosis and its clinical surgical guidance value.Method The clinical data of 99 patients with chest wall tuberculosis admitted to our hospital from January 2015 to June 2018 were retrospectively analyzed.The results of the MSCT exa mination were observed for diagnostic analysis and postoperative treatment effect.Results Among the 99 patients,81 cases with chest wall mass were found by MSCT,accounting for 81.82%,65 cases with cartilage and bone destruction were found by MSCT,accounting for 65.66%,43 cases with calcification in lesion were found by MSCT,accounting for 43.43%;37 cases with’crab-like’sinus were found by MSCT,accounting for 37.37%and 35 cases with rib damage were found by MSCT,accounting for 35.35%.The lesions were located in the soft tissue of the chest wall in 51 cases.In 37 cases,the abscess cavity appea red in the intercostal and in the muscle,and there were sinus tracts to connect in the middle,showing dumbbell-shaped lesions.Calcification occurred a round the liquefaction a rea and in the wall of the abscess cavity in 43 patients.The wound of all patients was healed in the first stage,and no recurrence occurred in 3 years of follow-up.Conclusion MSCT examination can clearly show the imaging features of chest wall tuberculosis.The relationship between the relevant tissue lesion can provide correct guidance for clinical,surgical treatment,effectively improve patients’quality of life,and is worth promoting.
