高龄产妇妊娠晚期心电图变化与妊娠结局的关系

Relationship between the Changes of Electrocardiogram in Late Pregnancy of Senile Parturients and Pregnancy Outcome

目的:探讨高龄产妇妊娠晚期心电图变化与妊娠结局的关系。方法:纳入2017年1月-2019年1月在医院建立档案的妊娠晚期产妇300例,按照预产期时产妇的年龄将产妇分为对照组(年龄<35岁,151例)和研究组(年龄≥35岁,149例)。产前对全部产妇行心电图检查,比较两组产妇心电图结果,统计分娩结局,比较两组心电图正常与心电图异常患者的分娩结局。结果:研究组心电图异常率高于对照组(P<0.05)。两组比较,研究组顺产率低于对照组(P<0.05),研究组妊娠结局不良率高于对照组(P<0.05)。两组心电图正常的产妇顺产率、早产率、低体质量儿率比较,差异无统计学意义(P>0.05);对于心电图异常的产妇,研究组顺产率低于对照组(P<0.05),研究组早产率、低体质量儿率高于对照组(P<0.05)。结论:高龄产妇妊娠晚期心电图异常的发生率及不良妊娠结局发生率较高,且其心电图异常变化与妊娠结局有关,临床应密切关注高龄产妇妊娠晚期心电图变化情况。

Objective:To explore the relationship between electrocardiogram changes in late pregnancy and pregnancy outcome of senile parturients.Method:A total of 300 cases of late-pregnancy women who were filed in the hospital from January 2017 to January 2019 were enrolled.The women were divided into the control group (age<35 years,151 cases) and the study group (age≥35 years,149 cases) according to their age at the expected delivery date.They were examined by electrocardiogram before delivery.The results of electrocardiogram in the two groups were compared,the outcomes of delivery were counted,and the outcomes of delivery of patients with normal electrocardiogram and abnormal electrocardiogram were compared between the two groups.Result:The abnormal rate of electrocardiogram in the study group was higher than that of the control group (P<0.05).The natural labour rate of the study group was lower than that in the control group (P<0.05).The rate of adverse pregnancy outcome in the study group was higher than that in the control group (P<0.05).For parturients with abnormal electrocardiogram,there were no significant differences in the rate of natural labour,rate of premature birth and rate of low birth weight newborn between the two groups (P>0.05).For parturients with abnormal ECG,the natural labour rate of in the study group was lower than that in the control group (P<0.05),the rate of premature birth and low body weight newburn in the study group was higher than that in the control group (P<0.05).Conclusion:The incidence of abnormal electrocardiogram in late pregnancy and adverse pregnancy outcomes is relatively high in elder parturients,and the abnormal changes in electrocardiogram are related to pregnancy outcome.Clinicians should pay close attention to the changes in electrocardiogram in late pregnancy.
