2019年某院临床药师参与224例会诊回顾性分析

Retrospective Analysis of 224 Cases of Clinical Pharmacists in a Hospital in 2019

目的:分析2019年临床药师参与药学会诊的相关数据和典型案例,为提高药学会诊质量提供参考。方法:收集2019年1-12月笔者所在医院临床药师参与224例会诊记录,调阅电子病历。结果:会诊科室以肿瘤内科、内分泌科、泌尿外科居多,会诊原因主要为原临床用药方案疗效不佳或病情反复需调整临床用药、协助制定初始诊疗方案,会诊用药以抗感染药物方面最集中,224例会诊中被采纳200例,占比89.28%,部分采纳和未被采纳均为12例,均占比5.36%。会诊结果为好转195例,占会诊病例数的87.05%,未愈自动出院或转院23例,占10.27%,因病情严重多脏器衰竭最后死亡6例,占2.68%。结论:临床药师的会诊意见在临床诊疗过程中发挥重要作用。

Objective:To analyze the relevant data and typical cases of clinical pharmacists participating in pharmaceutical consultation from January to December 2019,so as to provide reference for improving the quality of pharmaceutical consultation.Method:The records of 224 clinical pharmacists involved in consultation in our hospital from January to December 2019 were collected and the electronic medical records were reviewed.Result:Most of the consultation departments were oncology,endocrinology and urology,the main reasons for consultation were poor efficacy of the original clinical medication regimen or repeated need to adjust the clinical medication and assist in the formulation of the initial treatment regimen,drug consultation with anti-infection drugs had the most concentrated.Among the 224 cases of consultation,200 cases were accepted,accounting for 89.28%,and 12 cases were both partially accepted and not accepted,accounting for 5.36%.The result of consultation was improved in 195 cases,accounting for 87.05% of the number of consultation cases,automatic discharge or transfer in 23 cases,accounting for 10.27%,and death in 6 cases due to severe multiple organ failure,accounting for 2.68%.Conclusion:Consultation opinions of clinical pharmacists play an important role in clinical diagnosis and treatment.
