腹腔镜手术与开放手术治疗结直肠癌的远期效果和并发症对比

Comparison of Long-term Effects and Complications between Laparoscopic Surgery and Open Surgery in Colorectal Cancer

目的:对腹腔镜手术与开放手术在结直肠癌患者远期疗效及并发症中的干预效果进行比较。方法:选取本院2015年6月-2016年12月64例结直肠癌患者为调查样本,采用随机分组的方式分为腹腔镜组和开放手术组,各32例。分别给予腹腔镜手术治疗、开放手术治疗,对患者进行为期3年的随访,调查患者生存率、复发率,对患者手术并发症、手术治疗时间、术中出血量等指标。结果:腹腔镜组术中出血量、术后肛门排气时间、术后进食时间、下床时间、住院时间均优于对照组,差异有统计学意义(P<0.05);腹腔镜组手术时间长于开放手术,差异有统计学意义(P<0.05)。腹腔镜组并发症发生率为9.4%,低于开放手术组的37.5%,差异有统计学意义(P<0.05)。两组术后1年复发率、术后3年复发率、术后3年生存率比较差异无统计学意义(P>0.05)。结论:腹腔镜手术与开放手术治疗远期效果相似,腹腔镜手术能够减少手术创伤,更有利于患者康复。

Objective:To compare the long-term effects and complications of laparoscopic surgery and open surgery in patients with colorectal cancer.Method:A total of 64 patients with colorectal cancer in our hospital were selected as the survey samples,and they were randomly divided into the laparoscopic group and the open surgery group,32 cases in each group.They were given laparoscopic surgery and open surgery respectively.The patients were followed up for 3 years to investigate the survival rate and recurrence rate of patients,and the operation complications,operation treatment time,intraoperative blood loss and other indicators were statistically analyzed.Result:The intraoperative blood loss,postoperative anal exhaust time,postoperative eating time,getting out of bed time and hospitalization time in the laparoscopic group were shorter than those in the control group (P<0.05);the operation time in the laparoscopic group was longer than that in the open operation group,the difference was statistically significant (P<0.05).The incidence of complications was 9.4% in the laparoscopic group,which was lower than 37.5% in the open surgery group,the difference was statistically significant (P<0.05).There were no significant differences in 1-year postoperative recurrence rate,3-year postoperative recurrence rate and 3-year postoperative survival rate between the two groups (P>0.05).Conclusion:The long-term effect between the laparoscopic surgery and open surgery for colorectal cancer is similar.Laparoscopic surgery can reduce surgical trauma and is more conducive to the rehabilitation of patients.
