系统营养干预对鼻咽癌放疗患者营养状况及放疗毒副反应的影响

Effects of systemic nutrition intervention on nutritional status and side effects of patients with nasopharyngeal carcinoma receiving radiotherapy

目的探讨系统营养干预对鼻咽癌放疗患者营养状况及放疗毒副反应的影响。方法选取2018年6月-2019年6月肿瘤科收治的鼻咽癌放疗患者88例,采用随机数字表将所有患者分为观察组及对照组,每组各44例,对照组放疗期间行常规饮食指导,观察组在对照组基础上实施系统营养干预,比较2组营养状况、放疗毒副反应情况及生活质量。结果干预后观察组血清前白蛋白、白蛋白、血红蛋白水平均高于对照组(t=5.628,P<0.001;t=3.822,P<0.001;t=8.498,P<0.001)。观察组放射性皮炎、口腔黏膜炎、唾液腺损伤等级均低于对照组(Z=-2.724,P=0.006;Z=-3.261,P=0.001;Z=-3.095,P=0.002)。干预后观察组身体功能、角色功能、情绪及记忆功能、放疗相关症状及生活质量总评分均高于对照组(P<0.001)。结论系统营养干预能有效改善鼻咽癌放疗患者营养状况,减轻患者放疗期间毒副反应,提高患者生活质量。

Objective To investigate the effect of systemic nutrition intervention on nutrition status and toxic side effects of patients with nasopharyngeal carcinoma.Methods Totally 88 patients undergoing radiotherapy for nasopharyngeal carcinoma in the oncology department between June 2018 and June 2019 were enrolled and randomly divided into an observation group(n=44)and a control group(n=44)according to a random number table.Both groups was given regular dietary guidance,while the observation group was additionally provided with the systematic nutrition intervention.The nutrition status,side effect and life quality of the two groups were compared.Results After the intervention,the level of serum pre-albumin,albumin and hemoglobin of the observation group were significantly higher than that of the control group(t=5.628,P<0.001;t=3.822,P<0.001;t=8.498,P<0.001).The incidence of radiation dermatitis,oral mucosal pharynx,salivary gland damage and otitis media of the observation group was significantly lower than that of the control group(Z=-2.724,P=0.006;Z=-3.261,P=0.001;Z=-3.095,P=0.002).The physical function,role function,mood and memory function and radiotherapy-related symptoms and life quality of the observation group were significantly higher than that of the control group(P<0.001).Conclusion Systemic nutrition intervention can effectively improve the nutrition status of patients with nasopharyngeal carcinoma after radiotherapy,reduce their side effect and improve their life quality.
