肺癌组织中VEGF、EGFR表达与ADC直方图相关参数的关系

Correlation between the expressions of VEGF,EGFR in lung cancer tissue and the parameters of ADC histogram

目的探讨肺癌表观弥散系数(apparent diffusion coefficient,ADC)直方图相关参数与肺癌组织中血管内皮生长因子(vascular endothelial growth factor,VEGF)、表皮生长因子受体(epidermal growth factor receptor,EGFR)表达量及淋巴结转移的关系。方法收集包含较大实性成分并接受手术切除治疗的肺癌患者32例,患者均在术前接受肺部弥散加权成像(DWI)检查,包括3个b值,b=0、400、800 s/mm^(2)。绘制全病变ADC直方图,并测量其相关参数,包括峰度、偏度、熵、ADC平均数、ADC中位数、ADC标准差、病变体积和ADC第5、10、15、30、50、70、90百分位数(ADC5th、ADC10th、ADC15th、ADC30th、ADC50th、ADC70th、ADC90th)。对肺癌组织切片分别进行VEGF和EGFR免疫组化染色,测定其表达量。分析ADC直方图相关参数与VEGF和EGFR表达量及淋巴结转移的相关性。结果VEGF表达量与各ADC直方图相关参数均无显著相关性(P>0.05);而EGFR表达量与ADC平均数、中位数、标准差、ADC50th、ADC70th、ADC90th呈显著负相关(P均<0.05);VEGF、EGFR表达量及直方图的峰度均与淋巴结转移显著相关(P值分别为0.001、0.004、0.03)。结论ADC直方图相关参数可以用于肺癌组织中EGFR水平及淋巴结转移的预测,具有一定临床应用价值。

Objective To investigate the correlation between the parameters of lung cancer apparent diffusion coefficient(ADC)histogram and the expressions of vascular endothelial growth factor(VEGF),epidermal growth factor receptor(EGFR)as well as lymph node metastasis in lung cancer.Methods A total of 32 patients with lung cancer who were treated by surgery removal were enrolled in the study.All the patients received lung diffusion weighted imaging(DWI)examination before surgery,including three b values,b=0,400,800s/mm^(2).And the ADC histograms of all lesions were drawn and the related parameters were measured,including kurtosis,skewness,entropy,ADC mean value,ADC median,ADC standard deviation,lesion volume,and the ADC percentile of 5th,10th,15th,30th,50th,70th,90th(ADC90th).Lung cancer tissue sections were immunohistochemical stained for VEGF and EGFR,and the expression levels were detected.The correlation among ADC histogram related parameters and the expression levels of VEGF,EGFR as well as lymph node metastasis was analyzed.Results There was no significant correlation between the expression levels of VEGF and the parameters of ADC histogram(P>0.05).However the expression levels of EGFR were significantly negatively correlated with ADC mean,median,standard deviation,ADC50th,ADC70th and ADC90th(P<0.05).Moreover the expression levels of VEGF,EGFR and the kurtosis of ADC were significantly correlated with the lymph node metastasis(P<0.01 or P<0.05).Conclusion The related parameter of ADC histogram can be used in predicting the EGFR levels and lymph node metastasis in lung cancer,which has certain clinical application value.
