早期活动对ICU机械通气患者谵妄的临床干预研究

Clinical Intervention of Early Activity on Delirium in ICU Patients with Mechanical Ventilation

目的:研究早期活动对ICU机械通气患者谵妄的临床干预。方法:选取本院2019年5-12月ICU机械通气患者90例,以随机数字表法分为两组,参照组给予常规护理干预,观察组给予早期活动干预,比较两组入ICU不同时间段内谵妄发生率、谵妄持续时间、谵妄发生频率,以及患者活动依从性、患者对临床干预满意度。结果:入ICU 8 h内,两组谵妄发生率比较,差异无统计学意义(P>0.05);入ICU 48 h内、出ICU前,观察组谵妄发生率低于参照组,差异有统计学意义(P<0.05);观察组谵妄持续时间短于参照组,谵妄发生频率低于参照组,差异有统计学意义(P<0.05);观察组活动依从性高于参照组,对临床干预满意度高于参照组,差异有统计学意义(P<0.05)。结论:早期活动干预用于ICU机械通气患者中,能够有效降低谵妄发生率,也能够缩短谵妄持续时间,降低谵妄发生频率,临床干预效果更显著,患者活动依从性与满意度更高,值得推广应用。

Objective:To study the clinical intervention of early activity on delirium in ICU patients with mechanical ventilation.Method:A total of 90 patients with mechanical ventilation in ICU of our hospital from May to December 2019 were selected and randomly divided into two groups.The reference group was given routine nursing intervention,and the observation group was given early activity intervention.The incidence rate of delirium,duration of delirium,frequency of delirium,activity compliance of patients and satisfaction of patients with clinical intervention were compared between the two groups.Result:Within 8 hours after admission to ICU,there was no significant difference in the incidence of delirium between the two groups(P>0.05).Within 48 hours and before leaving ICU,the incidence of delirium in the observation group were lower than those in the reference group,the differences were statistically significant(P<0.05);the duration of delirium in the observation group was shorter than that in the reference group,and the frequency of delirium was lower than that in the reference group,the differences were statistically significant(P<0.05);the activity compliance of the observation group was higher than that of the reference group,and the satisfaction of clinical intervention was higher than that of the reference group(P<0.05).Conclusion:Early activity intervention in ICU patients with mechanical ventilation,can effectively reduce the incidence of delirium,but also can shorten the occurrence time of delirium,reduce the frequency of delirium,clinical intervention effect is more significant,patient activity compliance and satisfaction is higher,it is worthy of promotion and application.
