胃镜下药物注射治疗消化性溃疡合并上消化道出血的临床效果

Clinical Effect Analysis of Drug Injection under Gastroscope in Treatment of Peptic Ulcer Combined with Upper Gastrointestinal Bleeding

目的:分析胃镜下药物注射在消化性溃疡伴发上消化道出血中的临床价值。方法:将2018年1月-2019年12月于重庆市东南医院消化内科就医的300例消化性溃疡合并上消化道出血患者根据随机数字表法分为观察组(n=150)和对照组(n=150)。对照组采用常规治疗,观察组基于常规治疗下加胃镜下药物注射治疗。比较两组临床疗效、各项临床指标及治疗前后胃液pH值。结果:观察组治疗总有效率(97.33%)高于对照组(85.33%),差异有统计学意义(P<0.05);观察组止血时间、疼痛停止时间均早于对照组,住院时间短于对照组,输血量少于对照组,差异均有统计学意义(P<0.05);治疗后24、72 h,两组pH值均高于治疗前,且观察组胃液pH值均高于对照组,差异均有统计学意义(P<0.05)。结论:胃镜下药物注射治疗能显著提高患者临床疗效,缩短住院时间、止血时间,有效缓解疼痛,降低输血量,改善患者的胃液pH值。

Objective:To analyze the clinical value of drug injection under gastroscope in peptic ulcer with upper gastrointestinal bleeding.Method:A total of 300 patients with peptic ulcer and upper gastrointestinal bleeding who were treated in our hospital from January 2018 to December 2019 were divided into the observation group(n=150)and the control group(n=150)according to the random number table.Patients in the control group received conventional treatment,and the observation group was treated with gastroscopic drug injection based on conventional treatment.The curative effect,various clinical indicators,and the gastric fluid pH value before and after treatment were compared between the two groups.Result:The total effective rate in the observation group(97.33%)was higher than that in the control group(85.33%),the difference was statistically significant(P<0.05).The time of hemostasis and pain cessation in the observation group was earlier than that in the control group,the length of hospital stay was shorter than that in the control group,and the amount of blood transfusion was less than that in the control group,the differences were statistically significant(P<0.05).The gastric fluid pH values of both groups were higher than before treatment at 24 and 72 h after treatment,and the gastric fluid pH value in the observation group were higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion:Drug injection therapy under gastroscopy can significantly improve the clinical efficacy of patients,shorten hospital stay,hemostasis time,effectively relieve pain,reduce blood transfusion,and improve the pH value of patients’gastric fluid.
