多囊卵巢综合征患者外周血T淋巴细胞群体分析 被引量:9

Analysis of T lymphocyte population in peripheral blood of patients with polycystic ovary syndrome

摘要 目的:探讨多囊卵巢综合征(PCOS)患者外周血是否有淋巴细胞亚群的变化,提示免疫因素是否与PCOS的发病与发展有关。方法:选取38例PCOS患者作为实验组,10例正常女性作为实验对照组。分别检测各组血浆中睾酮、雌二醇、FSH、LH水平。流式分析外周血各T细胞亚群及细胞因子的分布情况。结果:PCOS患者血浆中睾酮,FSH和LH的分泌明显增多,差异具有统计学意义(P<0.05)。雌二醇水平的差异并无统计学意义。FSH与LH的比值上升。PCOS组外周血CD3^(+)总T细胞比例明显下降,CD4^(+)T细胞占比上升,CD8^(+)T细胞占比下降。CD4/^(+)CD8^(+)T细胞比例上升。PCOS患者血浆中Th1型细胞因子IFN-γ明显上升,IL-2和TNF-α有所上调,但差异无统计学意义。结论:CD4^(+)T细胞在PCOS的发生与发展过程中发挥重要作用。IFN-γ可能是PCOS临床诊断的有效鉴别指标。PCOS患者体内可能存在Th1/Th2平衡的紊乱。 Objective:To investigate whether there are changes in lymphocyte subsets in peripheral blood of patients with poly⁃cystic ovary syndrome(PCOS),suggested whether immune factors are related to incidence and development of PCOS.Methods:38 PCOS patients were selected as experimental group,10 normal women subjects were used as experimental control group.Levels of tes⁃tosterone,estradiol,FSH and LH in plasma were measured respectively.Flow cytometric analysis to detected T cell subsets and cyto⁃kines in peripheral blood.Results:The secretion of testosterone,FSH and LH in plasma of PCOS patients increased significantly,and the difference was statistically significant(P<0.05).Difference in estradiol levels was not statistically significant.Ratio of FSH to LH increased.In PCOS group,proportion of CD3^(+)T cells in peripheral blood decreased significantly,proportion of CD4^(+)T cells increased,and proportion of CD8^(+)T cells decreased.Proportion of CD4/^(+)CD8^(+)T cells increased.In PCOS patients,Th1-type cytokine IFN-γin⁃creased significantly,and IL-2 and TNF-αwere up-regulated,but the difference was not statistically significant.Conclusion:CD4^(+)T cells play an important role in the occurrence and development of PCOS.IFN-γmay be an effective differential index in clinical diagno⁃sis of PCOS.Disturbance of Th1/Th2 balance may exist in patients with PCOS.
