血清AFP、CEA水平与乙肝肝硬化患者HBV-DNA载量的关系研究

Relationship between Serum AFP, CEA Levels and HBV-DNA Load in Patients with Hepatitis B Cirrhosis

目的:探究血清AFP、CEA水平与乙肝肝硬化患者HBV-DNA载量的关系。方法:选取2019年1-6月本院收治的乙肝肝硬化患者618例作为研究组,按照病毒载量分为低病毒载量组(<1.0×10^(3) IU/mL)、中病毒载量组(1.0×10^(3)~1.0×10^(5) IU/mL)、高病毒载量组(>1.0×10^(5) IU/mL),各206例,同时选取同时期到本院进行健康体检的618例健康人员作为对照组。对比研究组和对照组的血清AFP、CEA水平,研究组不同病毒载量的血清AFP、CEA水平以及血清AFP、CEA水平与患者HBV-DNA载量的相关性。结果:研究组的血清AFP、CEA水平均高于对照组(P<0.05);低病毒载量组的血清AFP、CEA水平均高于对照组(P<0.05),中病毒载量组的血清AFP、CEA水平均高于低病毒载量组(P<0.05),高病毒载量组的血清AFP、CEA水平均高于中病毒载量组(P<0.05)。研究组的HBV-DNA载量与AFP水平呈正相关(r_(s)=0.550,P<0.05),与CEA水平无明显相关性(r_(s)=0.040,P=0.695)。结论:乙肝肝硬化患者的血清AFP、CEA水平在不同载量之间存在明显的差异性。另外血清AFP水平与HBV-DNA载量具有明显的相关性,临床上可以依据血清AFP水平对乙肝肝硬化患者进行临床诊断,为随后的临床治疗提供比较有价值的参考依据。

Objective:To explore the relationship between serum AFP,CEA levels and HBV-DNA load in patients with hepatitis B cirrhosis.Method:A total of 618 patients with hepatitis B cirrhosis admitted to our hospital from January 2019 to June 2019 were selected as the study group.According to the viral load,they were divided into low viral load group(<1.0×10^(3) IU/mL),medium viral load group(1.0×10^(3)-1.0×10^(5) IU/mL),high viral load group(>1.0×10^(5) IU/mL),206 cases in each group,at the same time,618 healthy people who came to our hospital for physical examination during the same period were selected as the control group.Serum AFP and CEA levels of the study group and the control group were compared,serum AFP and CEA levels of different viral loads in the study group and the correlation between serum AFP and CEA levels and HBV-DNA load of patients were compared.Result:Serum AFP and CEA levels in the study group were higher than those in the control group(P<0.05).Serum AFP and CEA levels in the low viral load group were higher than those in the control group(P<0.05).Serum AFP and CEA levels in the medium viral load group were higher than those in the low viral load group(P<0.05).Serum AFP and CEA levels in the high viral load group were higher than those in the medium viral load group(P<0.05).In the study group,HBV-DNA load was positively correlated with AFP level(r_(s)=0.550,P<0.05),and had no significant correlation with CEA level(r_(s)=0.040,P=0.695).Conclusion:The levels of AFP and CEA in patients with hepatitis B cirrhosis are significantly different among different loads.In addition,serum AFP level is significantly correlated with HBV-DNA load,which can be used for clinical diagnosis of hepatitis B cirrhosis patients according to serum AFP level,providing a valuable reference for subsequent clinical treatment.
