老年2型糖尿病患者尿白蛋白/肌酐比值与血清补体C3、C4的相关性分析

Correlation Analysis of Urinary Albumin/Creatinine Ratio and Serum Complement C3 and C4 in Elderly Patients with Type 2 Diabetes Mellitus

目的:研究2型糖尿病患者尿白蛋白/肌酐比值(ACR)与血清补体C3、C4关系。方法:选取2019年6月-2020年6月本院收治的107例2型糖尿病患者为研究对象,按照UAER情况分为单纯糖尿病组(A组:UAER<20 μg/min)30例、早期糖尿病肾病组(B组:20 μg/min≤UAER≤200 μg/min)46例、糖尿病肾病组(C组:UAER>200 μg/min)31例。另选择同期本院收治的老年健康体检者37例为对照组。比较各组的各项生化指标,分析ACR与各项指标的相关性。结果:各组生化指标水平比较,差异均有统计学意义(P<0.05)。A、B、C组的SBP、DBP、FPG、HbA1c、TC、TG、LDL-C、ACR、C4、CRP水平均明显高于对照组,C3、HDL-C水平明显低于对照组(P<0.05);B、C组的SBP、DBP、FPG、ACR、CRP水平均明显高于A组,C3水平明显低于A组(P<0.05);C组SBP、ACR、CRP水平均明显高于B组,C3水平均明显低于B组(P<0.05)。经Pearson相关性分析,老年2型糖尿病患者ACR与SBP、CRP均呈正相关,与血清C3呈负相关(P<0.05)。结论:2型糖尿病患者ACR与血清补体C3存在密切相关,与补体C4无相关性,其中补体C3可能参与了糖尿病肾病的发生、发展过程。

Objective:To study the relationship between urinary albumin/creatinine ratio(ACR)and serum complement C3 and C4 in patients with type 2 diabetes mellitus.Method:A total of 107 patients with type 2 diabetes mellitus admitted to our hospital from June 2019 to June 2020 were selected as the research objects.According to the condition of UAER,they were divided into three groups,simple diabetic group(group A,UAER<20μg/min)30 cases,early diabetic nephropathy group(group B,20μg/min≤UAER≤200μg/min)46 cases,diabetic nephropathy group(group C,UAER>200μg/min)31 cases.In addition,37 healthy aged persons admitted to our hospital during the same period were selected as the control group.The biochemical indexes of each group were compared,and the correlation between ACR and each index was analyzed.Result:There were statistically significant differences in biochemical indexes among all groups(P<0.05).The levels of SBP,DBP,FPG,HbA1c,TC,TG,LDL-C,ACR,C4 and CRP in group A,B and C were significantly higher than those in control group,and the levels of C3 and HDL-C were significantly lower than those in control group(P<0.05).The levels of SBP,DBP,FPG,ACR and CRP in group B and C were significantly higher than those in group A,and the level of C3 was significantly lower than that in group A(P<0.05).The levels of SBP,ACR and CRP in group C were significantly higher than those in group B,and the level of C3 in group C were significantly lower than those in group B(P<0.05).According to Pearson correlation analysis,ACR was positively correlated with SBP and CRP,and negatively correlated with serum C3 in elderly patients with type 2 diabetes mellitus(P<0.05).Conclusion:ACR is closely correlated with serum complement C3,but not with complement C4 in patients with type 2 diabetes mellitus,among them,complement C3 may be involved in the occurrence and development of diabetic nephropathy.
