老年2型糖尿病患者发生肌少症的影响因素分析

Analysis of influencing factors of sarcopenia in elderly patients with type 2 diabetes mellitus

目的分析老年2型糖尿病(diabetes mellitus of type 2,T2DM)患者发生肌少症的影响因素。方法选取2018年1月至2020年1月解放军联勤保障部队第900医院收治的老年T2DM患者130例,根据欧洲老年肌少症工作组(European Working Group on Sarcopenia in Older People,EWGSOP)诊断标准,分为T2DM合并肌少症组(50例)、T2DM非肌少症组(80例),比较两组的基本临床资料,采用单因素及多因素分析影响老年2型糖尿病患者发生肌少症的相关因素。结果T2DM合并肌少症组的体质量、HDL-C、TG低于T2DM非肌少症组,HbA1c、APN、IL-6高于T2DM非肌少症组,差异有统计学意义(P<0.05);多因素logistic回归分析显示,HbA1c(OR=3.792,95%CI:1.443~9.967,P=0.007)、IL-6(OR=3.516,95%CI:1.364~9.068,P=0.009)是老年T2DM患者发生肌少症的相关影响因素。结论HbA1c、IL-6可能是老年T2DM患者发生肌少症的相关影响因素,对T2DM合并肌少症的临床诊疗有潜在的指导意义。

Objective To analyze the influencing factors of sarcopenia in elderly patients with diabetes mellitus of type2(T2DM).Methods One hundred and thirty patients with T2DM were selected from the 900 Hospital of the Joint Logistics Team from January 2018 to January 2020.According to the European Working Group on Sarcopenia in Older People(EWGSOP)diagnostic criteria,patients were divided into T2DM with sarcopenia group(n=50)and T2DM non-sarcopenia group(n=80).The basic clinical data of the two groups were compared and the related factors of sarcopenia in elderly patients with T2DM by single factor and multiple factors were analyzed.Results In terms of clinical data of the two groups,body weight index,HDL-C and TG of the T2DM with sarcopenia group were lower than those of the T2DM non-sarcopenia group.HbA1 c,APN and IL-6 were higher than those of the T2DM non-sarcopenia group.Logistic regression analysis showed that HbA1 c(OR=3.792,95%CI:1.443-9.967,P=0.007)and IL-6(OR=3.516,95%CI:1.364-9.068,P=0.009)were related factors of sarcopenia in elderly T2DM patients.Conclusions HbA1 c and IL-6 may be related factors influencing the occurrence of sarcopenia in elderly T2DM patients and have potential guiding significance for the clinical diagnosis and treatment of T2DM with sarcopenia.
