静脉注射右美托咪定辅助全身麻醉的价值分析

Analysis of the Value of Intravenous Injection of Dexmedetomidine in Assisting General Anesthesia

目的探讨静脉注射右美托咪定辅助全身麻醉对阻滞及镇静效果的影响。方法选择2019年5月—2020年4月该院拟于全身麻醉下行外科手术治疗的106例患者为研究对象,以随机数字表分组法将患者分为研究组与对照组,研究组予以右美托咪定静脉注射辅助全身麻醉,对照组则注射等量生理盐水作为参照,比较两组阻滞与镇静效果。结果研究组感觉、运动阻滞起效时间分别为(9.5±2.4)min、(7.1±1.3)min,均较对照组(21.8±3.7)min、(13.5±2.1)min的感觉、运动阻滞时间显著缩短,差异有统计学意义(t=20.304、18.864,P<0.05);研究组阻滞维持时间为(234.8±23.4)min,较对照组(217.9±15.6)min的阻滞维持时间延长,差异有统计学意义(t=4.374,P<0.05)。随着用药时间的延长,两组镇静评分均呈增加趋势,其中研究组用药后20、60 min的Ramsay评分均较对照组高,差异有统计学意义(t=11.848、21.200,P<0.05)。结论全身麻醉辅助应用右美托咪定取得了良好效果,患者应用丙泊酚等麻醉药物的剂量明显减少,运动与感觉的阻滞起效时间缩短,阻滞维持时间延长,且镇静效果也得以进一步提升。

Objective To investigate the effect of intravenous injection of dexmedetomidine assisting general anesthesia on the effect of block and sedation.Methods From May 2019 to April 2020,106 patients planned to undergo surgical treatment under general anesthesia in the hospital were selected as the research object.The patients were divided into study group and control group by random number table grouping method,and the study group was given dexamethasone tomidine was injected intravenously to assist general anesthesia,and the control group was injected with the same amount of saline as a reference to compare the blocking and sedative effects of the two groups.Results The onset time of sensory and motor block in the study group was(9.5±2.4)min and(7.1±1.3)min,respectively,which were shorter than those of the control group(21.8±3.7)min and(13.5±2.1)min.The difference was statistically significant(t=20.304,18.864,P<0.05);the retention time of block in the study group was(234.8±23.4)min,which was longer than that of the control group(217.9±15.6)min,the difference were statistically significant(t=4.374,P<0.05).With the extension of the medication time,the sedation scores of the two groups showed an increasing trend.Among them,the Ramsay scores of the study group were higher than those of the control group at 20 min and 60 min after medication,the difference was statistically significant(t=11.848,21.200,P<0.05).Conclusion The adjuvant application of general anesthesia with dexmedetomidine has achieved good results.The dosage of propofol and other anesthetics in patients was significantly reduced,the onset time of motor and sensory block was shortened,the block maintenance time was prolonged,and the sedative effect was also achieved further improvement.
