基于RCA分析对呼吸内科老年住院患者跌倒风险的干预评价

Intervention Evaluation of Fall Risk for Elderly Hospitalized Patients in Department of Respiratory Medicine Based on RCA Analysis

目的探讨RCA分析对呼吸内科老年住院患者跌倒风险的干预价值。方法2019年1—6月该院实施常规护理的老年呼吸内科住院患者636例作为对照对象(常规组),2019年7—12月进行RCA分析干预的老年呼吸内科住院患者601例作为观察对象(RCA组),常规组实施常规防跌倒护理;RCA组实施跌倒发生根本原因分析、防跌倒护理对策。观察指标两组护士对防跌倒知识考试成绩。患者及家属对防跌倒措施配合情况,老年跌倒危险人群跌倒发生率,护理满意度。结果RCA组织护士防跌倒知识考试成绩(96.37±2.56)分,陪伴人员防跌倒措施护理94.51%、护理满意率99.33%高于常规组的(86.09±4.98)分、78.30%、95.28%,差异有统计学意义(t=4.234,χ^(2)=32.091、8.973,P<0.05);RCA跌倒发生率0.00%低于常规组0.63%,差异有统计学意义(P<0.05)。结论分析呼吸内科老年住院患者跌倒发生的原因和高发因素,通过护理干预措施能够达到控制和减少患者跌倒的发生。

Objective To explore the intervention value of RCA analysis on the risk of falls in elderly hospitalized patients in the Department of Respiratory Medicine.Methods From January to June 2019,636 elderly patients in the Department of Respiratory Medicine who performed routine care in the hospital were used as control subjects(conventional group),and 601 elderly patients in the Department of Respiratory Medicine who underwent RCA analysis intervention from July to December 2019 were used as observation subjects(RCA group),the routine group implemented routine fall prevention care;root cause analysis of falls,implementation of fall prevention nursing countermeasures,observation index nurses’test scores on fall prevention knowledge;patient and family members’cooperation with fall prevention measures,fall incidence rate of elderly people at risk of falling,nursing satisfaction.Results RCA organized nurses'fall prevention knowledge test scores(96.37±2.56)points and accompanying staff fall prevention measures nursing 94.51%,nursing satisfaction rate 99.33%,higher than the conventional group(86.09±4.98)points,78.30%,95.28%,the difference was statistically significant(t=4.234,χ^(2)=32.091,8.973,P<0.05);the incidence of RCA falls at 0.00%was lower than 0.63%in the conventional group,the difference was statistically significant(P<0.05).Conclusion Analyze the causes and high-risk factors of falls in elderly hospitalized patients in the Department of Respiratory Medicine.Nursing interventions can control and reduce the occurrence of falls in patients.
